摘　　要：目的探讨产程中胎心宫缩曲线图在宫内胎儿窘迫的早期诊断价值。方法选择住院孕妇153例,根据胎心监护图形将分娩胎儿分为胎儿窘迫组(n=58)和对照组(n=95)。比较胎儿窘迫组与对照组分娩情况、新生儿结局、不同种类异常胎心宫缩曲线图血气分析差异性。结果胎儿窘迫组剖宫产率和产钳率均高于对照组(P<0.05);胎儿窘迫组新生儿窒息率高于对照组(P<0.05);胎儿窘迫组重复复杂变异减速、频发晚期减速脐动脉血气分析pH、BE均低于对照组(P<0.05)。结论胎心监护异常图形特征重复复杂变异减速及频发晚期减速与胎儿窘迫关系较为密切。Objective To explore the early diagnostic value of fetal heart rate and uterine contraction curve diagram during labor in intrauterine fetal distress.Methods 153 hospitalized pregnant women were selected,and the delivered fetuses were divided into the fetal distress group(n=58)and the control group(n=95)according to fetal heart monitoring graph.Delivery status,neonatal outcome,and the difference of blood gas analysis of different types of abnormal fetal heart rate and uterine contraction curve diagram between the fatal distress group and the control group were compared.Results The cesarean section rate and forceps delivery rate in fetal distress group were higher than those in control group(P<0.05).The incidence of neonatal asphyxia in the fetal distress group was higher than that in the control group(P<0.05).Umbilical artery blood gas analysis showed that pH and BE of repeated complex variant deceleration and frequent late deceleration in the fetal distress group were lower than those in the control group(P<0.05).Conclusion Abnormal patterns in fetal heart monitoring,especially repeated complex variant deceleration and frequent late deceleration,are closely related to fetal distress.
