超声测量子宫动脉血流指标结合血清丝氨酸蛋白酶抑制剂B1及妊娠相关蛋白对早期妊娠不良结局的预测价值

摘　　要：目的探究超声测量子宫动脉血流指标结合血清丝氨酸蛋白酶抑制剂B1(serpinB1)、妊娠相关蛋白A(PAPP-A)对早期妊娠不良结局的预测价值。方法回顾分析2022年9月至2024年1月在南阳市中心医院建档接受产检的64例早期妊娠不良结局孕妇与74名健康妊娠的孕妇临床资料,分别纳入不良组和正常组。比较2组一般资料、子宫动脉血流指标其血清serpinB1、PAPP-A差异。分析早期妊娠不良结局的影响因素,并验证子宫动脉血流指标结合血清serpinB1、PAPP-A的预测价值。结果不良组阻力指数(0.64±0.09与0.72±0.11;t=-4.698,P=<0.001)、搏动指数(2.49±0.14与2.69±0.26;t=-5.729,P<0.001)、serpinB1[(2.2±0.5)ng/ml与(2.8±0.7)ng/ml;t=-5.766,P<0.001]高于正常组,PAPP-A[(107±35)mU/ml与(62±22)mU/ml;t=8.858,P<0.001]、平均血流速度[(21.3±1.6)cm/s与(19.0±1.4)cm/s;t=8.819,P<0.001]低于正常组。平均血流速度[OR值(95%CI)=0.312(0.120,0.816),P=0.019]、阻力指数[OR值(95%CI)=1.811(1.147,2.860),P=0.012]、搏动指数[OR值(95%CI)=1.680(1.320,2.138),P<0.001]、serpinB1[OR值(95%CI)=1.767(1.145,2.724),P=0.011]、PAPP-A[OR值(95%CI)=0.203(0.071,0.583),P=0.003]均为早期妊娠不良结局影响因素(P<0.05);且经受试者工作特征(ROC)曲线验证均存在早期妊娠不良结局预测价值(P<0.05)。结论超声测量子宫动脉血流指标结合血清serpinB1、PAPP-A可用于预测早期妊娠不良结局。Objective To explore the predictive value of ultrasound measurement of uterine artery blood flow indicators combined with serum serine protease inhibitor B1(serpinB1)and pregnancy associated protein(PAPP-A)in early pregnancy adverse outcomes.Methods The clinical data of 64 pregnant women with adverse early pregnancy outcome and 74 pregnant women with normal pregnancy were retrospectively analyzed and included into the adverse group and the normal group respectively.in the Nanyang Central Hospital from Sep.2022 to Jan.2024.The differences of serum serpinB1 and PAPP-A,in general data,uterine artery blood flow indexes were compared between the two groups.To analyze the influencing factors of early pregnancy adverse outcome,and to verify the predictive value of uterine artery blood flow index combined with serum SerpinB1 and PAPP-A.Results Resistance index of adverse group(0.64±0.09 vs.0.72±0.11;t=-4.698,P<0.001),pulsation index(2.49±0.14 vs.2.69±0.26;t=-5.729,P<0.001),and serpinB1 of adverse group were higher than normal group[(2.2±0.5)ng/ml vs.(2.8±0.7)ng/ml;t=-5.766,P<0.001].PAPP-A[(107±35)mU/ml vs.(62±22)mU/ml;t=8.858,P<0.001],The the mean blood flow velocity[(21.3±1.6)cm/s vs.(19.0±1.4)cm/s;t=8.819,P<0.001]was lower than that of normal group.Mean blood flow velocity[OR(95%CI)=0.312(0.120,0.816),P=0.019],resistance index[OR(95%CI)=1.811(1.147,2.860),P=0.012],pulse index[OR(95%CI)=1.680(1.320,2.138),P<0.001],serpinB1[the OR(95%CI)=1.767(1.145,2.724),P=0.011],PAPP-a[OR(95%CI)=0.203(0.071,0.583),P=0.003]adersewere influencing factors for adverse outcome of early pregnancy factor(P<0.05).The ROC curve verified that all of them had predictive value of early pregnancy adverse outcome(P<0.05).Conclusion Ultrasound measurement of uterine artery blood flow indicators combined with serum serpinB1 and PAPP-A can be used to predict adverse outcomes in early pregnancy.
