摘　　要：提高老年人出院准备服务的质量是促进医养结合、改善医疗服务质量的重要方法。本文对T/CAWAORG 004—2023《老年患者出院准备服务指南》这一团体标准进行了深入解读。本标准的主要内容包括开展出院准备服务的一般原则及实施理念、开展机构的类型、开展人员的类型、服务的内容以及服务的流程。其中,本标准特别指出了开展这一服务的核心人员——个案管理员,开展过程中的关键技术——老年综合评估技术,以及服务决策中容易被忽视的利益相关者——患者和照护者。此外,本标准建议形成“出院准备服务清单”,用于标准化开展服务项目,控制服务质量。老年患者出院准备服务是延续护理的重要组成部分。本文为采标单位及实施老年出院准备服务的各级医疗机构及相关医务人员提供技术参考。Improving the quality of hospital discharge preparation services for the elderly is an important issue in promoting the development of healthcare integration and improving the quality of healthcare services.This paper provides an in-depth interpretation of the organization standard T/CAWAORG 004—2023“Guidelines for the Development of Discharge Planning among Elderly Inpa-tients”.The main contents of the standard include general principles and implementation concepts for carrying out discharge prepara-tion services,types of organizations carrying out the service,types of personnel carrying out the service,contents of the service,and the process of the service.In particular,the standard identifies the case manager as the core person who develops the service,the comprehensive geriatric assessment as a key skill,and the patient and caregiver as the overlooked stakeholders in the decision-making process.In addition,the standards recommend the development of a“Discharge Preparation Checklist”as a tool to standardize the delivery of the service and to control the quality of the service.Preparation of elderly patients for hospital discharge is an important tool for continuity of care.This article is intended to be a technical reference for organizations planning to develop discharge prepara-tion services for the elderly.
