TyG指数与2型糖尿病患者大血管并发症的相关性研究

摘　　要：目的探讨2型糖尿病患者三酰甘油葡萄糖乘积指数(TyG指数)与2型糖尿病患者大血管并发症的相关性。方法回顾性分析2022年1月至2023年12月于上海市嘉定区安亭医院内分泌科及外冈和黄渡社区卫生服务中心就诊的253例2型糖尿病患者资料,根据脉搏波传导速度(PWV)是否大于14m/s分为PWV正常组和PWV升高组,根据TyG指数按照四分位数分为Q1、Q2、Q3、Q4组,比较各组资料的差异,采用ROC曲线评价TyG指数对大血管并发症的预测作用。结果PWV升高组年龄、糖尿病病程、HbA1c、AST、Cr、TyG指数、TG、TC、晨尿白蛋白-肌酐显著高于PWV正常组,差异有统计学意义(P<0.05);HDL-C、TIR显著低于PWV正常组,差异有统计学意义(P<0.05)。Pearson相关性分析表明PWV与TyG指数、年龄、糖尿病病程、AST、TG、HDL-C、TIR具有相关性(r=0.234,0.436,0.253,0.126,0.209,-0.124,-0.145,P<0.05)。结论2型糖尿病患者存在糖脂代谢异常,TyG指数升高增加2型糖尿病大血管并发症发生风险。Objective To investigate the correlation between the triacylglycerol glucose product index(TyG index)and macro-vascular complications in patients with type 2 diabetes mellitus.Methods The data of 253 patients with type 2 diabetes mellitus who attended the endocrinology department of Anting hospital in Jiading district,Shanghai and the Community Health Service Center of Waigang and Huangdu from January 2022 to December 2023 were retrospectively analyzed,and divided into the PWV normal group and the elevated group according to whether the PWV was greater than 14 m/s,and divided into the Q1,Q2,Q3 and Q4 groups according to the quartile according to the TyG index to compare the differences of particulars among the groups,and the ROC curve was used to evaluate the role of TyG index in predicting macrovascular complications.Results Age,duration of diabetes mellitus,HbA1c,AST,Cr,TyG index,TG,TC,and morning urine albumin-creatinine were significantly higher in the PWV elevated group than in the PWV normal group,with statistically significant differences(P<0.05);and HDL-C and TIR were significantly lower than in the PWV normal group,with statistically significant differences(P<0.05).Pearson correlation analysis showed that PWV was corre-lated with TyG indices,age,duration of diabetes,AST,TG,HDL-C and TIR(r=0.234,0.436,0.253,0.126,0.209,-0.124,-0.145,P<0.05).There was a correlation between PWV and TyG index,age,duration of diabetes,AST,TG,HDL-C,TIR(r=0.234,0.436,0.253,0.126,0.209,-0.124,-0.145,P<0.05).Conclusion Abnormalities of glucose and lipid metabolism are present in patients with type 2 diabetes mellitus,and elevated TyG index increases the risk of macrovascular complications in type 2 diabetes mellitus.
