自拟活血通络生骨汤治疗早期激素性股骨头坏死临床研究

摘　　要：目的 分析自拟活血通络生骨汤治疗早期激素性股骨头坏死的临床疗效。方法 选取96例早期激素性股骨头坏死患者,根据随机数字表法将其分为对照组和观察组,每组48例。对照组给予阿仑膦酸钠对症治疗,观察组在对照组基础上增加自拟活血通络生骨汤。治疗3个月后,比较2组患者临床总有效率、髋关节功能评分、血液流变学指标、骨代谢指标及不良反应。结果 治疗后,观察组临床总有效率明显高于对照组(P<0.05);观察组Harris髋关节功能评分量表各维度评分均高于对照组(P均<0.05);观察组血液流变学指标均低于对照组(P均<0.05);观察组碱性磷酸酶、骨γ-羧基谷氨酸蛋白水平均高于对照组(P均<0.05)。不良反应发生率组间比较,差异无统计学意义(P>0.05)。结论 自拟活血通络生骨汤治疗早期激素性股骨头坏死可调节骨代谢水平,改善微血管循环,促进髋关节功能的恢复,并具有一定的安全性。Objective To analyze the clinical efficacy of self-made Huoxue Tongluo Shenggu decoction in the treatment of early steroid-induced femoral head necrosis.Methods Ninety-six patients with early steroid-induced femoral head necrosis were selected and randomly divided into the control group and the observation group using a random number table method,with 48 cases in each group.The control group was given symptomatic treatment with alendronate sodium,while the observation group was given the self-made Huoxue Tongluo Shenggu decoction on the basis of the control group.After 3 months of treatment,the clinical total effective rate,hip joint function scores,hemorheological indicators,bone metabolism indicators and adverse reactions were compared in the two groups.Results After treatment,the clinical total effective rate of the observation group was higher than that of the control group(P<0.05).The scores of each dimension of Harris hip function scale in the observation group were higher than those in the control group(all P<0.05).The levels of hemorheological indicators in the observation group were lower than those in the control group(all P<0.05).The levels of alkaline phosphatase and boneγ-carboxyglutamic acid protein in the observation group were higher than those in the control group(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Self-made Huoxue Tongluo Shenggu decoction could regulate bone metabolism levels in the treatment of early steroid-induced femoral head necrosis,improve microvascular circulation,and promote the recovery of hip joint function,which is with certain safety.
