益气健脾补肾方治疗维持性血液透析肾性贫血疗效观察

摘　　要：目的:观察益气健脾补肾方治疗维持性血液透析肾性贫血的临床疗效。方法:将62例符合诊断标准的血液透析患者随机分为两组各31例。对照组予常规西医治疗及皮下注射促红细胞生成素;治疗组在对照组基础上予益气健脾补肾方治疗,疗程均为12周。观察两组患者血红蛋白、透前血清肌酐、中医证候积分、促红细胞生成素的用量及不良反应发生率。结果:治疗组总有效率(83.87%%)优于对照组(25.81%),两组比较差异有统计学意义(P﹤0.05)。治疗后,治疗组的血红蛋白水平明显高于对照组,且促红细胞生成素的用量明显少于对照组,差异均具有统计学意义(P﹤0.05);治疗组透前血清肌酐水平与中医证候积分均明显低于对照组(P﹤0.05)。治疗组不良反应发生率(67.74%)明显低于对照组(12.90%),差异具有统计学意义(P﹤0.05)。结论:中西医结合疗法在血液透析患者肾性贫血脾肾气虚证的治疗中具有明显疗效,能有效改善中医证候,提升肾功能指标,降低不良反应发生率。Objective:To observe the clinical efficacy of Yiqi Jianpi Bushen formula on maintenance hemodialysisinduced renal anemia.Methods:62 cases of eligible patients with hemodialysis were randomly divided into two groups,31 cases in each group.The control group was treated with routine western medicine and subcutaneous injection of erythropoietin,while the treatment group was treated plus with Yiqi Jianpi Bushen formula on the basis of control group.The course of treatment was 12 weeks for both groups.Hemoglobin,pre-hemodialysis serum creatinine,TCM syndrome score,dosage of erythropoietin and incidence rate of adverse reactions of patients were observed in two groups.Results:The total effective rate of treatment group was 83.87%,which was significantly higher than 25.81%of control group,and the comparative difference between two groups was statistically significant(P﹤0.05).After treatment,the hemoglobin level of treatment group was significantly higher than that of control group,and the erythropoietin dosage of treatment group was significantly lower than that of control group,and the differences were statistically significant(P﹤0.05);the serum creatinine level and TCM syndrome score of treatment group before dialysis were significantly lower than those of control group(P﹤0.05);the incidence rate of adverse reactions of treatment group was 6.774%,which was significantly lower than 12.90%of control group,and the difference was statistically significant(P﹤0.05).Conclusion:The integrated therapy of traditional Chinese and western medicine has significant clinical efficacy on maintenance hemodialysis-induced renal anemia with syndrome of deficiency of spleen qi and kidney qi,which can effectively improve TCM syndromes,enhance the indexes of renal function and reduce the incidence rate of adverse reactions.
