腹腔镜食管裂孔疝修补联合胆囊切除术的临床疗效

摘　　要：目的分析腹腔镜食管裂孔疝修补联合胆囊切除术的临床疗效。方法选取徐州医科大学附属医院2014年10月至2023年11月行腹腔镜食管裂孔疝修补术联合胆囊切除术的36例患者,纳入观察组,选取同期行腹腔镜食管裂孔疝修补术的36例患者,纳入对照组。分析比较两组患者的临床指标、术前和术后3个月的症状评分(食物反流、胸痛)、炎症指标[白介素-6(IL-6)、白介素-1β(IL-1β)、C反应蛋白(CRP)]及并发症。结果两组术中出血量、术后住院时间相比无统计学差异(P>0.05),观察组手术时间长于对照组(P<0.05);术前,两组食物反流、胸痛评分无统计学差异(P>0.05),两组术后3个月的症状评分均降低,且观察组低于对照组(P<0.05);术前,两组IL-6、IL-1β、CRP水平无统计学差异(P>0.05);术后3个月,两组以上炎症指标水平低于对照组(P<0.05)。观察组的并发症率低于对照组(8.33%比27.78%,P<0.05)。结论腹腔镜食管裂孔疝修补术联合胆囊切除术治疗食管裂孔疝的临床疗效显著,可以明显改善患者的临床症状,在无明显禁忌证的情况下安全可行。Objective To analyze the clinical effect of laparoscopic hiatal hernia repair combined with cholecystectomy.Methods A total of 36 patients who underwent laparoscopic hiatus hernia repair combined with cholecystectomy in the Department of General Surgery,Affiliated Hospital of Xuzhou Medical University from October 2014 to November 2023 were collected as the observation group,and 36 patients who underwent laparoscopic hiatus hernia repair during the same period were selected as the control group.The clinical indexes,symptom scores(food reflux,chest pain),inflammatory indexes[interleukin-6(IL-6),interleukin-1β(IL-1β),C-reactive protein(CRP)]and complications of the two groups were analyzed and compared.Results There were no differences between the two groups in the amount of intraoperative blood loss and postoperative hospital stay(P>0.05),and the operative time of the observation group was longer than that of the control group(P<0.05).Before surgery,there were no differences in the scores of food reflux and chest pain between the two groups(P>0.05).Symptom scores of the two groups were decreased 3 months after surgery,and the observation group was lower than the control group(P<0.05).Before operation,there were no differences in IL-6,IL-1βand CRP levels between the two groups(P>0.05).Three months after operation,the levels of inflammation indexes in two groups were lower than those in control group(P<0.05).The complication rate of the observation group was 8.33%,lower than that of the control group(27.78%,P<0.05).Conclusion Laparoscopic repair of hiatus hernia combined with cholecystectomy in the treatment of hiatus hernia has significant clinical efficacy,can significantly improve the clinical symptoms of patients,and is safe and feasible in the absence of obvious contraindications.
