参苓白术散联合双歧杆菌三联活菌片治疗小儿脾虚型功能性便秘的疗效

摘　　要：目的探究参苓白术散联合双歧杆菌三联活菌片治疗小儿脾虚型功能性便秘的疗效。方法前瞻性选择2021年3月至2024年6月165例于榆林市第一医院治疗的脾虚型功能性便秘患儿,经纳入排除标准筛选后,最终纳入140例患儿作为研究对象。采用电脑随机分配法将患儿分成对照组(双歧杆菌三联活菌片治疗,n=70)与研究组(参苓白术散联合双歧杆菌三联活菌片治疗,n=70),对比两组临床疗效、主症积分、中医证候积分、治疗前后患儿血清一氧化氮(NO)及肠神经递质P物质(SP)、不良反应发生情况(皮疹、恶心、呕吐、腹泻)及复发情况。结果研究组治疗总有效率(87.14%)显著高于对照组(71.43%)(P<0.05);治疗后研究组与对照组主症积分、中医证候积分均显著降低,且研究组下降幅度更为显著(P<0.05);治疗后两组NO降低,SP升高,且研究组的变化幅度更大(P<0.05);研究组(12.86%)与对照组(15.71%)不良反应率差异无统计学意义(P>0.05);研究组复发率(8.57%)显著低于对照组(18.57%)(P<0.05)。结论参苓白术散联合双歧杆菌三联活菌片对小儿脾虚型功能性便秘有显著的治疗效果,可有效改善其排便情况及胃肠功能,降低复发的可能性,安全性较好。Objective To explore the curative effect of Shenling Baizhu Powder combined with bifidobacterium triplet on functional constipation with spleen deficiency in children.Methods A total of 165 children with functional constipation with spleen deficiency treated in First Hospital of Yulin from March 2021 to June 2024 were prospectively selected,and 140 children were finally included as study objects after screening with inclusion and exclusion criteria.The children were divided into control group(bifidobacterium triple viable tablet treatment,n=70)and study group(Shenling Baizhu Powder combined with bifidobacterium triple viable tablet treatment,n=70)by computer random allocation method.The clinical efficacy,main symptom score,traditional Chinese medicine(TCM)syndrome score,serum nitric oxide(NO)and the intestinal neurotransmitter substance P(SP),occurrence of adverse reactions(rash,nausea,vomiting,diarrhea)and recurrence of the two groups were compared.Results The total effective rate of the study group(87.14%)was significantly higher than that of the control group(71.43%)(P<0.05).After treatment,the main disease score and TCM syndrome score of the study group and the control group were significantly decreased,and the decreased of the study group was more significant(P<0.05).After treatment,NO decreased and SP increased in the two groups,and the changes in the study group were greater(P<0.05).There was no significant difference in the adverse reaction rate between the study group(12.86%)and the control group(15.71%)(P>0.05).The recurrence rate of study group(8.57%)was significantly lower than that of control group(18.57%)(P<0.05).Conclusion Shenling Baizhu Powder combined with bifidobacterium triplet has significant therapeutic effect on functional constipation of spleen deficiency type in children,which can effectively improve defecation and gastrointestinal function,reduce the possibility of recurrence,and has good safety,which can be used for clinical promotion and application.
