摘 要:目的:研究干燥综合征(sjögren′s syndrome,SS)患者外周血中低氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)的表达水平,分析HIF-1α的表达水平与SS患者实验室指标和口干症状的相关性。方法:选取64例SS患者和64例健康体检者。PCR法检测两组外周血单个核细胞(peripheral blood mononuclear cell,PBMC)中HIF-1αmRNA的表达水平;ELISA法检测血清中HIF-1α蛋白的表达水平;检测SS患者的免疫球蛋白(immunoglobulin,Ig)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、白细胞(white blood cell,WBC)、血红蛋白(hemoglobin,Hb)等指标。结果:与健康对照组相比,SS患者PBMC中HIF-1αmRNA的表达水平增加(P<0.05);SS患者血清中HIF-1α的表达水平与ESR、IgG、IgM呈正相关(r_(1)=0.4576、r_(2)=0.4269、r 3=0.4039,P<0.05),与WBC、Hb呈负相关(r_(1)=-0.3333、r_(2)=-0.3125,P<0.05)。口干阳性SS患者的HIF-1α蛋白的表达水平高于口干阴性者(P<0.01)。ROC曲线分析HIF-1αmRNA和蛋白的表达水平对SS的诊断价值,AUC分别为0.6594和0.8134,敏感度分别为64.1%和76.6%,特异度分别为95.3%和81.2%。结论:SS患者外周血中HIF-1α的表达水平增高,与ESR、IgG、IgM呈正相关,与WBC、Hb呈负相关,SS患者中口干阳性患者血清中HIF-1α的表达水平高于阴性者,说明HIF-1α对SS有一定的诊断价值,并在SS发病中发挥作用。Objective:To analyze the correlation between the expression level of hypoxia inducible factor-1α(HIF-1α)and relevant laboratory indicators and dry mouth symptoms in sjögren's syndrome(SS)patients by comparing the expression levels of HIF-1αin peripheral blood between healthy individuals and patients with SS.Methods:A total of 64 SS patients and 64 health examiners were selected.PCR was used to detect the expression level of HIF-1αmRNA in peripheral blood mononuclear cells(PBMC)of the two groups.ELISA method was used to detect the expression level of HIF-1αprotein in serum.Immunoglobulin(Ig),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),white blood cell(WBC),hemoglobin(Hb)and other indicators from SS patients were collected.Results:Compared with the healthy control group,the expression levels of HIF-1αmRNA in PBMC were increased(P<0.05).HIF-1αexpression level in serum of SS patients was positively correlated with ESR,IgG and IgM(r_(1)=0.4576,r_(2)=0.4269,r 3=0.4039,P<0.05),but negatively correlated with WBC and Hb(r_(1)=-0.3333,r_(2)=-0.3125,P<0.05.)The expression level of HIF-1 protein in positive dry mouth patients was higher than that in negative dry mouth patients(P<0.01).The diagnostic value of the expression level of HIF-1αmRNA and protein on SS was analyzed using ROC curves,and the AUC was 0.6594 and 0.8134,with the sensitivity of 64.10%and 76.60%,and the specificity of 95.30%and 81.20%,respectively.Conclusion:The expression level of HIF-1αmRNA and protein were higher in PBMC of SS patients.It is positively correlated with ESR,IgG and IgM,while negatively correlated with WBC and Hb.The expression level of HIF-1αin SS patients with positive dry mouth was higher than that of SS patients with negative dry mouth,indicating that HIF-1αhas certain diagnostic value in SS and plays a role in the occurrence of SS.
