经皮微创脊柱内固定系统治疗创伤性胸腰段骨折的临床效果分析

摘  要:目的探讨创伤性胸腰段骨折患者采取经皮微创脊柱内固定系统进行治疗的效果及其对患者炎症因子以及椎体功能的影响。方法72例创伤性胸腰段骨折患者,按随机数字表法分为对照组和研究组,每组36例。对照组以切开椎弓根螺钉内固定手术治疗,研究组患者实施经皮微创脊柱内固定系统治疗。对比两组患者Cobb角、伤椎前缘高度比值、疼痛情况、应激指标[皮质醇(Cor)、磷酸肌酸激酶(CK)、前列腺素E_(2)(PGE_(2))]水平、炎症因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)]水平。结果治疗后,研究组患者Cobb角(8.25±1.42)°低于对照组的(11.41±2.14)°,伤椎前缘高度比值(91.65±16.84)%高于对照组的(73.07±15.11)%,差异有统计学意义(P<0.05)。治疗后,研究组患者的疼痛评分(3.32±0.71)分低于对照组的(4.70±0.85)分,差异有统计学意义(P<0.05)。治疗后,研究组患者Cor(171.39±22.73)ng/ml、CK(178.64±19.14)U/L、PGE_(2)(82.01±21.67)μg/L均低于对照组的(200.88±30.53)ng/ml、(250.98±22.16)U/L、(95.47±26.46)μg/L,差异有统计学意义(P<0.05)。治疗后,研究组患者CRP(7.52±0.95)mg/L、TNF-α(40.03±6.91)pg/ml、IL-1β(20.02±3.10)pg/ml均低于对照组的(12.04±1.42)mg/L、(47.02±8.81)pg/ml、(25.02±4.25)pg/ml,差异有统计学意义(P<0.05)。结论经皮微创脊柱内固定系统对创伤性胸腰段骨折患者的治疗效果较好,可改善术后的应激反应、疼痛情况、炎症情况,帮助患者更好地恢复椎体功能,值得推广应用。Objective To explore the effect of minimally invasive percutaneous spinal internal fixation system in the treatment of patients with traumatic thoracolumbar fracture,and its influence on inflammatory factors and vertebral function of patients.Methods 72 patients with traumatic thoracolumbar fracture were divided into a control group and a study group according to random number table method,with 36 cases in each group.The control group was treated with open pedicle screw internal fixation surgery,and the study group was treated with minimally invasive percutaneous spinal internal fixation system.Both groups were compared in terms of Cobb angle,anterior height ratio of the injured vertebra,pain,stress index[cortisol(Cor),creatine kinase(CK),prostaglandin E_(2)(PGE_(2))]levels,inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)]levels.Results After treatment,the Cobb angle of(8.25±1.42)°in the study group was lower than(11.41±2.14)°in the control group,and the anterior height ratio of the injured vertebra of(91.65±16.84)% was higher than(73.07±15.11)% in the control group.The difference was statistically significant(P<0.05).After treatment,the pain score of(3.32±0.71)points in the study group was lower than(4.70±0.85)points in the control group,and the difference was statistically significant(P<0.05).After treatment,the study group had Cor of(171.39±22.73)ng/ml,CK of(178.64±19.14)U/L,and PGE_(2) of(82.01±21.67)μg/L,which were lower than(200.88±30.53)ng/ml,(250.98±22.16)U/L,and(95.47±26.46)μg/L in the control group,and the difference was statistically significant(P<0.05).After treatment,the study group had CRP of(7.52±0.95)mg/L,TNF-α of(40.03±6.91)pg/ml,and IL-1β of(20.02±3.10)pg/ml,which were lower than(12.04±1.42)mg/L,(47.02±8.81)pg/ml and(25.02±4.25)pg/ml in the control group,and the difference was statistically significant(P<0.05).Conclusion Minimally invasive percutaneous spinal fixation system has a good therapeutic effect on patients wit
