评价硫代硫酸钠与加巴喷丁联合治疗维持性血液透析患者皮肤瘙痒的效果

摘　　要：目的观察硫代硫酸钠与加巴喷丁联合治疗维持性血液透析(MHD)患者皮肤瘙痒的效果。方法选取50例维持性血液透析皮肤瘙痒患者作为研究对象,数字编号后随机分成观察组(硫代硫酸钠与加巴喷丁联合治疗)和对照组(硫代硫酸钠治疗),各25例。比较两组患者皮肤瘙痒程度,血清检测结果(血清肌酐、尿素氮、甲状旁腺激素、C反应蛋白、血钙、血磷),不良反应发生情况及治疗效果满意度。结果治疗前、治疗结束、停药后1周,观察组患者改良Duo氏瘙痒评分量表评分分别为(33.52±1.44)、(7.33±1.41)、(8.76±1.43)分,对照组分别为(33.55±1.39)、(13.53±1.39)、(15.76±1.46)分。治疗结束、停药后1周,两组患者改良Duo氏瘙痒评分量表评分较治疗前均显著降低,且观察组患者低于同一时间对照组(P<0.05)。治疗结束,两组患者血清肌酐、尿素氮、甲状旁腺激素、C反应蛋白、血钙、血磷较治疗前均明显改善,且观察组患者均优于对照组(P<0.05)。观察组患者不良反应发生率略高于对照组,但差异不明显(P>0.05)。观察组患者对治疗效果满意度96.00%显著高于对照组的76.00%(P<0.05)。结论维持性血液透析皮肤瘙痒患者采用硫代硫酸钠与加巴喷丁联合治疗效果显著优于硫代硫酸钠单独治疗。Objective To observe the effect of sodium thiosulfate combined with gabapentin on pruritus in patients on maintenance hemodialysis(MHD).Methods 50 patients with pruritus on maintenance hemodialysis were selected as the study subjects and randomly divided into observation group(combined treatment with sodium thiosulfate and gabapentin)and control group(combined treatment with sodium thiosulfate),each with 25 cases.Patients in both groups were compared in terms of degree of pruritus,serum test results(serum creatinine,urea nitrogen,parathyroid hormone,C-reactive protein,blood calcium,blood phosphorus),occurrence of adverse reactions and satisfaction with treatment effect.Results Before treatment,at the end of treatment and 1 week after drug withdrawal,the scores of modified Duo's VAG scale in the observation group were(33.52±1.44),(7.33±1.41)and(8.76±1.43)points,and those in the control group were(33.55±1.39),(13.53±1.39)and(15.76±1.46)points.At the end of treatment and 1 week after drug withdrawal,the scores of modified Duo's VAG scale in both groups were significantly lower than those before treatment,and the observation group was lower than the control group(P<0.05).At the end of treatment,the serum creatinine,urea nitrogen,parathyroid hormone,C-reactive protein,blood calcium and blood phosphorus in both groups were significantly improved compared with those before treatment,and the observation group was better than the control group(P<0.05).The incidence of adverse reactions in the observation group was slightly higher than that in the control group,but the difference was not significant(P>0.05).The satisfaction with treatment effect in the observation group was 96.00%,which was significantly higher than 76.00% in the control group(P<0.05).Conclusion The therapeutic effect of sodium thiosulfate combined with gabapentin is significantly better than that of sodium thiosulfate alone in patients with pruritus on maintenance hemodialysis.
