摘　　要：目的:观察以手三阳经为主行透穴刺法治疗中风后手指痉挛的临床疗效。方法:选取2023年3月—2024年3月于广东省第二中医院康复科治疗的72例中风后手指痉挛患者,按随机数字表法分为治疗组及对照组各36例。治疗组以手三阳经为主行透穴刺法,对照组给予常规针刺治疗。2组均治疗14 d。比较2组临床疗效,比较2组治疗前后改良Ashworth量表(MAS)分级、手Brunnstrom分级量表(BR-H)分级、简化FuglMeyer运动功能量表(FMA-UE)评分的变化。结果:治疗组总有效率为90.00%(27/30),高于对照组70.00%(21/30)(P<0.05)。治疗后,2组MAS、BR-H分级均较治疗前改善,治疗组MAS、BR-H分级均优于对照组(P<0.05);2组FMA-UE评分较治疗前升高(P<0.05),治疗组FMA-UE评分高于对照组(P<0.05)。结论:以手三阳经为主行透穴刺法治疗中风后手指痉挛,可有效降低手指肌张力、改善手指痉挛程度、提高手运动功能,临床疗效好。Objective:To observe the clinical effect of point penetration acupuncture mainly conducted on three yang channels of the hand on post-stroke finger spasms.Methods:A total of 72 patients with post-stroke finger spasms treated in the Rehabilitation Department of Guangdong Second Traditional Chinese Medicine Hospital from March 2023 to March 2024 were randomly divided into the treatment group and the control group,with 36 cases in each group,according to the random number table method.The treatment group received point penetration acupuncture mainly conducted on three yang channels of the hand,while the control group received conventional acupuncture treatment.Both groups were treated for 14 days.Compared the clinical effects,modified Ashworth Scale(MAS)grading,Hand Brunnstrom Rating Scale(BR-H)grading,and Simplified Fugl Assessment of Upper Extremity(FMA-UE)scores between the two groups.Results:The total effective rate was 90.00%(27/30)in the treatment group,which was higher than that of 70.00%(21/30)in the control group(P<0.05).After treatment,both MAS and BR-H grades improved compared with that before treatment,and MAS and BR-H grades were better than the control group(P<0.05).Both groups showed an increase in FMA-UE scores when compared with those before treatment(P<0.05),with the treatment group having higher FMA-UE scores than the control group(P<0.05).Conclusion:The point penetration acupuncture mainly conducted on three yang channels of the hand for post-stroke finger spasms can effectively reduce the finger muscle tension,improve the degree of finger spasms,and improve the hand motor function,with good clinical effect.
