健脾补肾逐瘀方联合火龙灸治疗脾肾亏虚夹瘀型糖尿病神经源性膀胱临床研究

摘　　要：目的:观察健脾补肾逐瘀方联合火龙灸治疗脾肾亏虚夹瘀型糖尿病神经源性膀胱(DNB)的疗效。方法:选择2021年10月—2022年11月临海市中医院收治的98例DNB患者,按随机数字表法分为联合组及对照组各49例。对照组口服坦索罗辛缓释胶囊,联合组在对照组基础上给予健脾补肾逐瘀方联合火龙灸治疗。比较2组临床疗效及安全性,比较2组治疗前后排尿功能、尿动力学指标[最大尿流率(MFR)、膀胱残余尿量(RUV)、膀胱最大容积]、中医证候积分、生活质量测评量表(Qualiveen)评分的变化。结果:联合组临床疗效总有效率为87.76%(43/49),对照组临床疗效总有效率为71.43%(35/49),2组比较,差异有统计学意义(P<0.05)。治疗后,2组日排尿次数均较治疗前减少(P<0.05),日均单次排尿量均较治疗前提升(P<0.05);联合组日排尿次数少于对照组(P<0.05),日均单次排尿量大于对照组(P<0.05)。治疗后,2组MFR、膀胱最大容积均较治疗前增加(P<0.05),RUV较治疗前下降(P<0.05);联合组MFR、膀胱最大容积均高于对照组(P<0.05);RUV低于对照组(P<0.05)。治疗后,2组中医证候积分、Qualivee评分均较治疗前下降(P<0.05),联合组中医证候积分、Qualivee评分均低于对照组(P<0.05)。结论:健脾补肾逐瘀方联合火龙灸治疗脾肾亏虚夹瘀型DNB疗效较好,可改善尿动力学指标值,提升排尿功能及生活质量。Objective:To observe the curative effect of Jianpi Bushen Zhuyu Prescription combined with fire-dragon moxibustion on diabetic neurogenic bladder(DNB)of spleen-kidney deficiency and stasis type.Methods:A total of 98 cases of DNB patients admitted to Linhai Hospital of Traditional Chinese Medicine from October 2021 to November 2022 were selected and divided into the combination group and the control group according to random number table method,with 49 cases in each group.The control group was orally treated with Tamsulosin Sustained-Release Capsules,and the combination group was given Jianpi Bushen Zhuyu Prescription combined with fire-dragon moxibustion based on the treatment of the control group.The clinical effects and safety in the two groups were compared,and the changes of urination function,urodynamic indexes[maximum urine flow rate(MFR),bladder residual urine volume(RUV),bladder maximum volume],traditional Chinese medicine syndrome scores and Qualiveen scores before and after treatment were compared between the two groups.Results:The total effective rate was 87.76%(43/49)in the combination group and 71.43%(35/49)in the control group,the difference being significant(P<0.05).After treatment,the daily frequency of urination in the two groups was decreased when compared with that before treatment(P<0.05),and the average daily single volume of urination was increased when compared with that before treatment(P<0.05);the number of daily urination in the combination group was lower than that in the control group(P<0.05),and the average daily single urination volume was more than that in the control group(P<0.05).After treatment,MFR and maximum bladder volume in the two groups were increased when compared with those before treatment(P<0.05),and RUV was decreased when compared with that before treatment(P<0.05);MFR and maximum bladder volume in the combination group were higher than those in the control group(P<0.05),and RUV was lower than that in the control group(P<0.05).After treatment,traditional Chinese medici
