摘　　要：间质性肺疾病(ILD)是一组病因复杂且异质性显著的慢性疾病,起病隐匿,多数患者确诊时已进入疾病进展期,预后较差,病死率高。现有诊断和治疗手段在疾病早期识别、动态监测及预后评估方面存在局限性。炎症标志物(如C反应蛋白、血清淀粉样蛋白A、白细胞介素-6)、纤维化标志物(如基质金属蛋白酶、透明质酸、Ⅱ型肺泡上皮细胞表面抗原)及新兴分子标志物(如循环微RNA、环状RNA)等在疾病活动性监测、急性加重预测及纤维化进程评估中显示出潜在价值。然而,这些标志物在不同ILD亚型中的特异性、敏感性及其与诊疗及预后间的关联仍存在争议。未来研究应聚焦于多重标志物联合应用的探索,并推进其在临床实践中的标准化和转化应用。Interstitial lung disease(ILD)is a group of chronic diseases with complex etiologies and significant heterogeneity.The onset of ILD is insidious,and most patients are already in the progressive stage at the time of diagnosis,leading to poor prognosis and high mortality.Current diagnostic and therapeutic methods have limitations in early detection,dynamic monitoring,and prognostic evaluation.Inflammatory markers(such as C-reactive protein,serum amyloid A,interleukin-6),fibrotic markers(such as matrix metalloproteinases,hyaluronic acid,type Ⅱ alveolar epithelial cell surface antigens),and emerging molecular biomarkers(such as circulating microRNA,circular RNA)have shown potential value in monitoring disease activity,predicting acute exacerbations,and assessing fibrotic progression.However,the specificity and sensitivity of these biomarkers,as well as their associations with diagnosis,treatment,and prognosis in different ILD subtypes,remain controversial.Future research should focus on exploring the combined application of multiple biomarkers and advancing their standardization and translational application in the clinical practice.
