从“血不利则为水”论治恶性心包积液

摘　　要：恶性心包积液,作为晚期恶性肿瘤的一种严重并发症,对患者的生活质量有着显著的不良影响,甚至危及生命。目前尚无有效的治疗药物及根治的方法。本病属于中医“心水”“支饮”等疾病范畴,且发病率较低,缺乏统一的认识及治疗方案。本文首次基于张仲景的“血不利则为水”的理论视角,探讨“血行不畅”与“水液代谢失调”之间的联系,并解析恶性心包积液形成的病理机制,明确了治疗恶性心包积液时“血利则水消”的辨证关系,并归纳目前临床上已取得成功的案例,为临床上治疗恶性心包积液提供新的思路,以期提高临床疗效。Malignant pericardial effusion,as a serious complication of advanced malignant tumors,has significant adverse effect on the quality of life of patients,even life-threatening.At present,there is no effective treatment and radical cure.This disease belongs to the category of“edema due to heart disorders”and“thoracic retained morbid fluid”in traditional Chinese medicine.The incidence of this disease is low,and there is a lack of unified understanding and therapeutic regime.Based on ZHANG Zhongjing’s theory of“stagnation of blood causing retention of water”,this paper discussed the relationship between“poor blood circulation”and“imbalance of water and fluid metabolism”,and analyzed the pathological mechanism of the formation of malignant pericardial effusion.The syndrome differentiation relationship of“blood benifited and water depleted”in the treatment of malignant pericardial effusion was clarified,and the successful cases in clinical practice were summarized,so as to provide new approaches for clinical treatment of malignant pericardial effusion and improve clinical efficacy.
