Superpath微创入路髋关节置换术对晚期股骨头坏死围术期指标及关节功能的影响

摘　　要：目的 探究Superpath微创入路行髋关节置换术(THA)治疗晚期股骨头坏死(ONFH)患者的效果。方法 选取2020年8月至2023年10月期间本院收治的84例行THA术的晚期ONFH患者进行前瞻性研究,依据随机数字表法分为A组(43例,PLA小切口入路)、B组(41例,Superpath微创入路)。两组均术后随访3个月。比较两组各项指标。结果 B组术中出血量更少,手术时间更长,首次下床行走时间、住院时间更短;24h大腿周径差异更小;术后3个月比之术前,A、B组FMA评分、Barthel指数、各项HHI评分均升高,B组更高;A、B组VAS评分均降低,B组更低,P<0.05;A、B组切口长度、术后3个月的并发症总发生率比较,无差异,P>0.05。结论 与PLA小切口入路,Superpath微创入路行THA术对晚期ONFH患者有助于改善围术期指标,促进运动功能、日常生活能力的恢复,降低疼痛程度,提高髋关节功能,安全性高,但手术时间长。Objective To Superpath minimally invasive approach for total hip arthroplasty(THA)in the treatment of patients with advanced osteonecrosis of femoral head(ONFH).Methods A prospective study was conducted on 84 patients with advanced ONFH who underwent THA in our hospital from August 2020 to October 2023.According to the random number table method,they were divided into the group A(43 cases,PLA small incision approach)and the group B(41 cases,Superpath minimally invasive approach).Both groups were followed up for 3 months.Compare the indicators of the two groups.Results In the group B,the amount of bleeding during operation was less,the operation time was longer,the first time to get out of bed and the time of hospitalization were shorter;the difference of thigh circumference in 24h was smaller.After operation for 3 months,compared with before treatment,the FMA score,Barthel index and HHI scores of the group A and the group B were higher,and the group B was higher.The VAS scores of the group A and the group B were lower,and the group B was lower,P<0.05.There was no difference in the length of incision and the total incidence of complications at 3 months after operation between the group A and the group B,P>0.05.Conclusion Compared with PLA small incision approach,Superpath minimally invasive approach for THA in patients with advanced ONFH is helpful to improve perioperative indicators,promote the recovery of motor function and daily living ability,reduce the degree of pain,improve the function of hip joint,and have high safety,but the operation time is long.
