天麻素注射液联合常规西药对高血压性眩晕临床症状及血压变异性的影响研究

摘　　要：目的 探究天麻素注射液联合常规西药对高血压性眩晕患者临床症状及血压变异性(BPV)的影响。方法 选择我院2021年1月至2022年6月收治高血压性眩晕患者50例,采用随机数表发分为对照组、实验组各25例。对照组行常规西药治疗,服用左氨氯地平,实验组在对照组基础上,注射天麻素注射液。于治疗前及治疗后2周评估其眩晕严重程度、频率、时间,并重点分析患者的24h平均收缩压和平均舒张压(24h-SBP/DBP)、日间平均收缩压和平均舒张压(d SBP/d DBP)、夜间平均收缩压和平均舒张压(n SBP/n DBP),同时以上记录述各时段(24h、日间及夜间3个时段)BPV指标,包括24h收缩压变异性(24h-SBPV)、24h舒张压变异性(24h-DBPV)、白昼收缩压变异性(d SBPV)、白昼舒张压变异性(d DBPV)、夜间收缩压变异性(n SBPV)、夜间舒张压变异性(n DBPV)。结果 两组高血压眩晕患者经过治疗后眩晕严重程度评分、眩晕频率、眩晕时间明显降低(P<0.05),且实验组降低程度高于对照组(P<0.05)。经治疗后,两组24h-SBP/DBP、d SBP/d DBP、n SBP/n DBP明显降低(P<0.05),24h-SBPV/DBPV、d SBPV/d DBPV、n SBPV/n DBPV亦明显降低(P<0.05),且实验组降低幅度均大于对照组(P<0.05)。结论 天麻素注射液联合常规西药可以有效高血压性眩晕患者症状,降低血压及改善BPV。Objective To investigate the effect of gastrodin injection combined with conventional western medicine on clinical symptoms and blood pressure variability(BPV)in patients with hypertensive vertigo.MethodsFifty patients with hypertensive vertigo who were admitted to the hospital from January 2021 to June 2022 were selected,and randomly divided into the control group and the treatment group,with 25 patients in each group.The control group was treated with routine western medicines(amlodipine).On this basis,the treatment group was treated with gastrodin injection.The severity,frequency and duration of vertigo were evaluated before treatment and after 2 weeks of treatment.24-hour mean systolic and diastolic blood pressure(24h-SBP/DBP),daytime mean systolic and diastolic blood pressure(dSBP/dDBP),nighttime mean systolic and diastolic blood pressure(nSBP/nDBP)were analyzed.BPV indicators[24-hour systolic blood pressure variability(24h SBPV)24-hour diastolic blood pressure variability(24-hour DBPV),daytime systolic blood pressure variability(dSBPV),daytime diastolic blood pressure variability(dDBPV),nighttime systolic blood pressure variability(nSBPV),and nighttime diastolic blood pressure variability(nDBPV)]were recorded.ResultsAfter treatment,the severity scores,frequency and duration of vertigo in the two groups were significantly reduced(P<0.05).The reduction in the treatment group was greater than that in the control group(P<0.05).After treatment,24-hour SBP/DBP,dSBP/dDBP,nSBP/nDBP,24-hour SBPV/DBPV,dSBPV/dDBPV,and nSBPV/nDBPV in the two groups were significantly reduced(P<0.05),and the reduction in the treatment group was greater than that in the control group(P<0.05).ConclusionThe combined treatment of gastrodin injection and conventional western medicines can effectively relieve symptoms,lower blood pressure,and improve BPV in patients with hypertensive vertigo.
