痔瘘倍硝熏洗散对湿热下注型环状混合痔微创术后患者肛缘水肿消退及创面愈合的影响

摘　　要：目的 探究痔瘘倍硝熏洗散对湿热下注型环状混合痔微创术后患者肛缘水肿消退及创面愈合的影响。方法选取2020年7月至2023年7月郑州人民医院收治的120例湿热下注型环状混合痔患者为研究对象,全部患者均行微创手术治疗,使用随机分组将全部120例患者分为观察组(n=60)和对照组(n=60),对照组予西医常规治疗,观察组在对照组的基础上使用痔瘘倍硝熏洗散治疗。评估两组患者手术1d、14d后肛缘水肿、创面愈合情况差异,分析两组患者术前、术后14d时肛肠动力学、血管生成指标水平变化,分析两组患者治疗期间药物不良反应差异。结果术后14d时,两组患者肛缘水肿、创面愈合情况评分均较术后1d时降低,且观察组肛缘水肿、创面愈合情况评分均低于对照组(P<0.05);术后14d时,两组患者肛管最大收缩压、高压区长度均较术前下降,且观察组低于对照组;两组患者直肠静息压及血管生成指标水平均较术前上升,且观察组直肠静息压及血管生成指标高于对照组(P<0.05)。结论痔瘘倍硝熏洗散对湿热下注型环状混合痔微创术后患者肛缘水肿消退效果明显,能促进患者创面愈合。Objective To explore the influence of Zhilou Beixiao fumigation powder on anal margin edema subsidence and wound healing in patients after minimally invasive surgery for circular mixed hemorrhoids of damp-heat flowing downward type.Methods Totally 120 patients with circular mixed hemorrhoids of damp-heat flowing downward type from July 2020 to July 2023 were selected as the research subjects.All patients were treated with minimally invasive surgery.According to the random grouping method,the above 120 patients were divided into observation group(n=60)and control group(n=60).The control group was treated with conventional western medicine,while the observation group was given Zhilou Beixiao fumigation powder on the basis of the control group.The differences in anal margin edema and wound healing were evaluated at 1 day and 14 days after surgery.The changes of anorectal dynamics and angiogenesis indexes before surgery and at 14 days after surgery and differences in adverse drug reactions during treatment were analyzed in both groups.Results At 14 days after surgery,the scores of anal margin edema and wound healing in the two groups were reduced than those at 1 day after surgery,and the scores in observation group were lower than those in control group(P<0.05).The maximum systolic pressure of anal canal and the length of high pressure area in the two groups at 14 days after surgery were declined compared with those before surgery,and the indexes in observation group were lower.The rectal resting pressure and levels of angiogenesis indexes were enhanced in both groups than those before surgery,and the observation group had higher indexes than those in control group(P<0.05).Conclusion Zhilou Beixiao fumigation powder has obvious effect on anal margin edema in patients after minimally invasive surgery for circular mixed hemorrhoids of damp-heat flowing downward type,and can promote the wound healing.
