脂肪组织基质血管组分移植在大鼠心肌缺血再灌注损伤中的作用研究

摘  要:目的 探究脂肪组织基质血管组分(SVF)静脉移植对大鼠心肌缺血再灌注损伤(MIRI)的治疗作用。方法 胶原酶法分离培养SD大鼠的SVF,36只雄性SD大鼠随机分为假手术组、缺血再灌注组、缺血再灌注+0.3×10^(6 )SVF/kg(治疗组1)、缺血再灌注+1×10^(6 )SVF/kg(治疗组2)、缺血再灌注+5×10^(6 )SVF/kg(治疗组3)和缺血再灌注+10×10^(6 )SVF/kg(治疗组4),每组6只;除假手术组外,其他各组大鼠均构建MIRI模型。采用小动物心动超声检测大鼠心脏功能,TTC染色评估大鼠心肌梗死情况,HE染色观察大鼠心肌组织病理损伤,Masson染色检测大鼠心肌组织纤维化情况,TUNEL染色评估大鼠心肌凋亡情况,免疫组化法评估大鼠心肌组织中CD31表达情况,Western blotting法检测大鼠心肌组织中TGF-β1表达情况。结果 与假手术组相比,缺血再灌注组大鼠心肌组织梗死面积明显增加,心脏收缩功能明显降低(P<0.01),心肌细胞排列紊乱、心肌纤维化程度明显增加(P<0.01),心肌细胞凋亡率明显上升,CD31表达减少,TGF-β1表达增加(P<0.01)。与缺血再灌注组相比,SVF治疗组大鼠心脏收缩功能明显改善(P<0.01),心肌细胞排列紊乱、心肌纤维化程度明显下降(P<0.01),心肌细胞凋亡率明显降低,CD31表达明显增加,TGF-β1表达减少(P<0.01),并呈现剂量依赖性。结论 静脉移植SVF可以降低心肌缺血再灌注大鼠心肌纤维化和心肌细胞凋亡程度,减轻心肌损伤,改善心脏功能,其机制可能与其促进心肌组织血管再生,抑制心肌组织中TGF-β1表达有关。Objective To investigate the therapeutic effect of intravenous transplantation of adipose tissue-derived stromal vascular fraction(SVF) on myocardial ischemia-reperfusion injury(MIRI) in rats.Methods SVF was isolated and cultured from SD rats by collagenase,and 36 male SD rats were randomly divided into sham-operated group,MIRI group,MIRI + 0.3 × 10^(6) SVF/kg(treatment group 1),MIRI + 1 × 10^(6) SVF/kg(treatment group 2),MIRI + 5 × 10^(6) SVF/kg(treatment group 3) and MIRI + 10 × 10^(6)SVF/kg(treatment group 4),with 6 rats in each group.MIRI model was constructed in all groups except the sham-operated group.Echocardiography was used to detect the cardiac function of rats,TTC staining was used to observe the area of myocardial infarction,HE staining was used to observe the pathological damage of myocardium,Masson staining was used to observe myocardial fibrosis,TUNEL staining was used to observe myocardial apoptosis,immunohistochemistry was used to observe CD31 expression in myocardial tissue,and Western blotting was used to detect the expression of TGF-β1 in myocardial tissues of rats.Results Compared with the sham-operated group,the area of myocardial infarction was significantly increased in MIRI group,the cardiac systolic function was markedly reduced(P<0.01),the degrees of cardiomyocyte disarrangement and myocardial fibrosis were observably increased(P<0.01),the rate of cardiomyocyte apoptosis was significantly increased,the expression of CD31 was decreased,and the expression of TGF-β1 was notably increased(P<0.01).Compared with the MIRI group,the cardiac systolic function was significantly improved in the SVF treatment group(P<0.01),the degrees of cardiomyocyte disarrangement and myocardial fibrosis were significantly decreased(P<0.01),the rate of cardiomyocyte apoptosis was significantly decreased,the expression of CD31 was significantly increased,and the expression of TGF-β1 was decreased(P<0.01) in a dose-dependent manner.Conclusion Intravenous transplantation of SVF can reduce the degrees of myoc
