摘 要:目的采用超声声触诊组织定量(VTQ)技术测量2型糖尿病(T2DM)患者胰腺硬度,探讨其与胰岛功能的关系。方法收集2020年6月至2021年1月空军军医大学唐都医院内分泌科住院的T2DM患者120(男89,女31)例,年龄(52.38±12.81)岁;糖尿病病程(7.82±6.28)年。所有受试者均进行人体学测量和空腹血糖、空腹C肽、糖化血红蛋白及其他生化指标检测。采用VTQ技术测量胰体硬度值。将患者按照胰体硬度三分位数进行分组,采用单因素方差分析法比较不同胰体硬度组的C肽水平;统计学相关分析采用Pearson相关性分析及多元逐步线性回归分析。结果随胰体硬度的增加,C肽水平逐渐下降(F=8.241,P<0.05);Pearson相关性分析显示胰体硬度值与糖尿病病程呈正相关(r=0.339,P<0.01),与C肽水平呈负相关(r=-0.308,P<0.01);多元线性回归分析显示,胰体硬度和糖尿病病程是C肽水平的独立影响因素(标准化回归系数β=-0.228,-0.051;P<0.05)。结论胰体硬度增加与C肽水平下降相关,临床中可采用VTQ技术评估胰腺硬度,并将胰体硬度作为衡量胰岛β细胞功能的指标。Objective To investigate the relationship between pancreatic stiffness and pancreatic islet function in patients with type 2 diabetes mellitus(T2DM) by using virtual touch tissue quantification(VTQ) technique.Methods A total of 120 hospitalized T2DM patients(89 males and 31 females),with a mean age of(52.38 ± 12.81) years and diabetes duration of(7.82 ± 6.28) years,were collected in Department of Endocrinology,Tangdu Hospital,Air Force Medical University from June 2020 to January 2021.All subjects underwent anthropometric measurements and fasting blood glucose,fasting C-peptide,glycosylated hemoglobin and other biochemical indicators.VTQ technique was used to measure the pancreatic stiffness.Patients were divided into groups according to the tertiles of pancreatic stiffness,and the C-peptide levels of different pancreatic stiffness groups were compared by one-way ANOVA.Pearson correlation analysis and multiple stepwise linear regression analysis were used for statistical correlation analysis.Results The C-peptide levels decreased gradually with the increase of pancreatic stiffness(F=8.241,P<0.05).Pearson correlation analysis showed that pancreatic stiffness was positively correlated with diabetes duration(r=0.339,P<0.01),and negatively correlated with C-peptide levels(r=-0.308,P<0.01).Multiple linear regression analysis showed that pancreatic stiffness and diabetes duration were independent influencing factors of C-peptide levels(standardized regression coefficients β=-0.228,-0.051,P<0.05).Conclusion Increased pancreatic stiffness is associated with decreased C-peptide levels.VTQ technique can be used to evaluate pancreatic stiffness,and pancreatic stiffness can be used as an indicator to measure pancreatic islet beta cell function in clinical practice.
