贵州地区某铅蓄电池厂铅接触人群血铅水平与外周血NSE和S100的相关性研究

摘  要:目的探讨贵州某铅蓄电池厂铅接触人群血铅与血清神经元特异性烯醇化酶(neuronen-spezifische enolase,NSE)、中枢神经特异蛋白(central neural specific protein,S100)水平的关系。方法采用电感耦合等离子体质谱仪(inductively coupled plasma mass spectrometry,ICP-MS)检测铅接触人群的血铅水平(n=149),根据血铅水平分为3组:<200μg/L、200~400μg/L、401~600μg/L。采用全自动化学发光法免疫分析仪检测受试者血清中神经元特异性烯醇化酶、中枢神经特异蛋白的水平。结果与男性比较,该厂女性血铅及血清NSE水平较低(P<0.01);在不同血铅水平组中,与低水平血铅组比较,高水平血铅组的人群接害工龄更长,NSE水平及男性占比较高(P<0.01);相关性分析结果显示,血铅水平与NSE、吸烟史呈正相关(rs=0.171、0.211,P均<0.05)、与S100、危险因素呈负相关(rs=-0.172、-0.177,P均<0.05)。结论血铅水平升高可引起血清NSE、S100变化,长期铅接触工人可定期通过检测血清NSE、S100水平而监测神经系统功能,为职业铅中毒的预防和治疗提供实验室依据。Objective To investigate the relationship between blood lead and serum neuronen-spezifische enolase(NSE)and central nervous system specific protein(S100)levels in lead-exposed population of a lead storage battery factory in Guizhou.Methods The blood lead concentration of the lead-exposed population(n=149)was detected by inductively coupled plasma mass spectrometry(ICP-MS),and these subjects were divided into three groups according to the blood lead concentration:<200μg/L,200-400μg/L,and 401-600μg/L.The serum concentrations of NSE and S100 were measured by a fully automated chemiluminescence method using immunoassay analyzer.Results Compared with males,females in the factory showed lower blood lead and serum NSE concentrations(P<0.01);Among the different blood lead concentration groups,compared with the low concentration group,the population in the high concentration group had longer years of work experience,higher NSE concentrations,and a higher proportion of males(P<0.01).The correlation analyses showed a positive correlation between the blood lead concentration and NSE,smoking history(r_s=0.171,0.211,P Both<0.05),while negatively correlated with S100 and risk factors(r_s=-0.172,-0.177,P<0.05).Conclusion Increasing blood lead concentration could cause changes in serum NSE and S100.Long-term lead-exposed workers should be monitored for their function of the nervous system by measuring the serum NSE and S100 concentrations regularly,which could provide a laboratory basis for the prevention and treatment of occupational lead poisoning.
