摘  要:目的探讨高通量与低通量血液透析对终末期肾病患者营养指标及微炎症指标的影响。方法选取2021年1—12月江西省萍乡矿业集团有限责任公司总医院收治的86例终末期肾病患者作为研究对象,根据随机数字表法分为低通量组(43例)与高通量组(43例)。低通量组采用低通量血液透析,高通量组采用高通量血液透析,两组均连续治疗6个月。比较两组患者治疗前后的肾功能、营养指标、微炎症指标状况。结果治疗后,两组患者的血肌酐(SCr)、尿素氮(BUN)水平均低于本组治疗前,且高通量组的SCr、BUN水平均低于低通量组,差异有统计学意义(P<0.05)。高通量组的尿素下降率(URR)高于低通量组,差异有统计学意义(P<0.05)。治疗后,两组患者的白蛋白(ALB)、转铁蛋白(TF)水平均高于本组治疗前,且高通量组的ALB、TF水平均高于低通量组,差异有统计学意义(P<0.05)。治疗后,两组患者的白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平均低于本组治疗前,且高通量组的IL-6、CRP、TNF-α水平均低于低通量组,差异有统计学意义(P<0.05)。结论与低通量血液透析治疗相比,高通量血液透析治疗更利于调节终末期肾病患者机体微炎症状态,改善机体肾功能及营养指标。Objective To explore the effect of high-flux and low-flux hemodialysis on nutritional indicators and micro-inflammatory indicators in patients with end-stage renal disease.Methods A total of 86 patients with end-stage renal disease admitted to the General Hospital of Pingxiang Mining Group Co.from January to December 2021 were selected as the research objects.According to the random number table method,they were divided into low-flux group(43 cases)and high-flux group(43 cases).The low-flux group was treated with low-flux hemodialysis,and the high-flux group was treated with high-flux hemodialysis.Both groups were treated for 6 months.The renal function,nutritional indicators and micro-inflammatory indicators of the two groups were compared before and after treatment.Results After treatment,the levels of serum creatinine(SCr)and blood urea nitrogen(BUN)in the two groups were lower than those before treatment,and the levels of SCr and BUN in the high-flux group were lower than those in the low-flux group,and the differences were statistically significant(P<0.05).The urea reduction ratio(URR)of the high-flux group was higher than that of the low-flux group,and the difference was statistically significant(P<0.05).After treatment,the levels of albumin(ALB)and transferrin(TF)in the two groups were higher than those before treatment,and the levels of ALB and TF in the high-flux group were higher than those in the low-flux group,and the differences were statistically significant(P<0.05).After treatment,the levels of interleukin 6(IL-6),C-reactive protein(CRP)and tumour necrosis factor-α(TNF-α)in the two groups were lower than those before treatment,and the levels of IL-6,CRP and TNF-αin the high-flux group were lower than those in the low-flux group,and the differences were statistically significant(P<0.05).Conclusion Compared with low-flux hemodialysis treatment,high-flux hemodialysis treatment is more conducive to regulating the micro-inflammatory state of patients with end-stage renal disease,improving renal funct
