摘　　要：目的分析Wiltse肌间隙入路手术治疗胸腰椎骨折的临床效果。方法选取2022年1月至2024年1月的鹰潭市一八四医院的90例胸腰椎骨折患者作为研究对象,采用随机数字表法将其分为传统组和研究组,每组各45例。传统组采用传统经皮入路手术治疗,研究组采用Wiltse肌间隙入路手术治疗。比较两组患者手术时间、术中出血量、术后切口疼痛评分、术后引流管放置时间、术后下地时间、住院时间、围手术期并发症发生率、术前及术后1个月伤椎前缘高度、腰椎功能。结果研究组手术时间长于传统组,研究组术中出血量、术后切口疼痛评分低于传统组,研究组术后引流管放置时间、术后下地时间、住院时间短于传统组,研究组围手术期并发症总发生率低于传统组,差异有统计学意义(P<0.05)。术后1个月,研究组伤椎前缘高度高于传统组,腰椎功能评分低于传统组,差异有统计学意义(P<0.05)。结论Wiltse肌间隙入路手术治疗胸腰椎骨折的临床效果显著,其出血少、疼痛轻、恢复快,可有效提高椎体前缘高度,改善腰椎功能,且并发症少。Objective To analyze the clinical efficacy of Wiltse intermuscular approach in the treatment of thoracolumbar fractures.Methods Ninety patients with thoracolumbar fractures from the 184th Hospital in Yingtan City from January 2022 to January 2024 were selected as the research subjects.They were divided into traditional group and study group according to the random number table method,with 45 cases in each group.The traditional group received traditional percutaneous approach surgery,while the study group received Wiltse intermuscular approach surgery.The surgical time,intraoperative blood loss,postoperative incision pain score,postoperative drainage tube placement time,postoperative down-to-earth time,hospital stay,perioperative complication rate,preoperative and one month postoperative vertebral anterior edge height,and lumbar spine function were compared between the two groups.Results The operative time of the study group was longer than that of the traditional group,the intraoperative blood loss and postoperative incision pain score of the study group were lower than those of the traditional group,the postoperative drainage tube placement time,postoperative time to the ground and hospital stay of the study group were shorter than those of the traditional group,and the total incidence of perioperative complications in the study group was lower than that of the traditional group,with statistical significances(P<0.05).One month after surgery,the frontal height of injured vertebrae in the study group was higher than the traditional group,the lumbar function scores was lower than the traditional group,with statistical significances(P<0.05).Conclusion The clinical effect of Wiltse intermuscular approach in the treatment of thoracolumbar fractures is significant,with less bleeding,less pain,faster recovery,effective improvement of anterior vertebral height,improvement of lumbar spine function,and fewer complications.
