交感神经阻滞改善骨质疏松脊柱压缩性骨折患者疼痛症状和腰背功能作用

摘　　要：目的 探讨交感神经阻滞改善骨质疏松脊柱压缩性骨折患者疼痛症状和腰背功能的作用。方法 选择2022年06月-2023年12月我院收治的骨质疏松脊柱压缩性骨折患者103例,按照随机数字表法分为观察组52例与对照组51例,对照组采取药物治疗等基础治疗方案,观察组在对照组的基础上增用交感神经阻滞,比较两组患者疼痛改善情况(包括各时间段视觉模拟评分情况)、睡眠质量改善情况(包括入院时、出院后14天匹兹堡睡眠指数评分情况)、腰背部功能改善情况(包括入院时、出院后14天Oswestry功能障碍指数问卷表评分情况)。结果 两组患者入院时及第4天阻滞前视觉模拟评分比较无统计学差异(P>0.05),观察组患者第4天阻滞后8h、第6天、第7天阻滞后8h、出院后14天视觉模拟评分低于对照组(P<0.05);两组患者入院时匹兹堡睡眠指数评分比较无统计学差异(P>0.05),观察组患者出院后14天匹兹堡睡眠指数评分低于对照组(P<0.05);两组患者入院时Oswestry功能障碍指数问卷表评分比较无统计学差异(P>0.05),观察组患者出院后14天Oswestry功能障碍指数问卷表评分低于对照组(P<0.05)。结论 药物等基础治疗联合交感神经阻滞术对骨质疏松脊柱压缩性骨折患者疼痛症状改善效果显著,且能很好的促进患者腰背功能恢复。Objective To investigate the effect of sympathetic nerve block on improving pain symptoms and low back function in patients with osteoporotic spinal compression fractures.Methods A total of 103 patients with osteoporotic spinal compression fracture admitted to our hospital from June 2022 December 2023 were selected and divided into an observation group(n=52)and a control group(n=51)according to a random number table.The control group was treated with basic treatment such as drug therapy,and sympathetic nerve block was added to the observation group on this basis.The improvement of pain(including visual analogue scale at each time period),sleep quality(including Pittsburgh Sleep Index score at admission and 14 days after discharge)and lower back function(including Oswestry Disability Index questionnaire score at admission and 14 days after discharge)were compared between the two groups.Results There was no significant difference in the visual analogue scale before block between the two groups at admission and on the 4th day(P>0.05).The visual analogue scale at 8h after block,8h after block on the 4th day,6th day,7th day and 14 days after discharge in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the Pittsburgh Sleep Index score at admission between the two groups(P>0.05).The Pittsburgh Sleep Index score at 14 days after discharge in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the Oswestry Disability Index questionnaire score at admission between the two groups(P>0.05).The Oswestry Disability Index questionnaire score at 14 days after discharge in the observation group was lower than that in the control group(P<0.05).Conclusions Basic treatment such as drugs combined with sympathetic nerve block is effective in improving pain symptoms in patients with osteoporotic spinal compression fracture,and can promote the recovery of low back function.
