摘 要:目的:探讨彩色多普勒血流成像(CDFI)在乳腺结节(BN)病变中的诊断价值。方法:选取济宁市兖州区中医医院2022年1月—2024年12月收治的92例BN患者,均行CDFI检查,并以穿刺活检为金标准,分析CDFI检出乳腺良恶性结节的情况及诊断价值,比较良恶性结节在收缩期流速峰值(PSV)、搏动指数(PI)、阻力指数(RI)、形态、边缘、钙化及内部回声方面的差异性。结果:92例BN患者经金标准检出恶性52例,良性40例;其中CDFI检出恶性51例,良性41例。CDFI诊断BN良恶性的灵敏度为96.15%(50/52)、特异度为97.50%(39/40)、准确率为96.74%(89/92)、阳性预测值为98.04%(50/51)、阴性预测值为95.12%(39/41),Kappa值为0.934,与病理结果高度一致;恶性结节PSV、PI、RI高于良性结节,差异有统计学意义(P<0.05);恶性结节形态不规则、边缘模糊、存在钙化灶、内部回声不均占比高于良性结节,差异有统计学意义(P<0.05)。结论:CDFI在BN良恶性诊断中的价值较高,可明确影像学特征上的差异,并获得血流动力学信息,利于早期定性诊断,实现针对性治疗。Objective To investigate the diagnostic value of color Doppler flow imaging(CDFI)in breast nodules(BN)lesions.Methods A total of 92 patients with BN admitted to Jining City Yanzhou District Hospital of Traditional Chinese Medicine between January 2022 and December 2024 were selected.All patients underwent CDFI examination,with puncture biopsy as the gold standard.The detection and diagnostic value of CDFI in distinguishing benign from malignant breast nodules were analyzed.Comparisons were made between benign and malignant nodules in terms of peak systolic velocity(PSV),pulsatility index(PI),resistance index(RI),morphology,margin,calcification,and internal echogenicity.Results Among the 92 BN patients,52 were confirmed as malignant and 40 as benign according to the gold standard.CDFI detected 51 malignant and 41 benign nodules.The sensitivity,specificity,accuracy,positive predictive value(PPV),and negative predictive value(NPV)of CDFI in diagnosing benign and malignant BNs were 96.15%(50/52),97.50%(39/40),96.74%(89/92),98.04%(50/51),and 95.12%(39/41),respectively,with a Kappa value of 0.934,indicating a high consistency with pathological results.PSV,PI,and RI were higher in malignant nodules than in benign ones,with statistically significant differences(P<0.05).Malignant nodules exhibited a higher proportion of irregular morphology,blurred margins,presence of calcifications,and heterogeneous internal echogenicity compared to benign nodules,with statistically significant differences(P<0.05).Conclusion CDFI has high value in differentiating benign from malignant BNs by identifying differences in imaging features and providing hemodynamic information,which facilitates early qualitative diagnosis and targeted treatment.
