摘  要:目的:利用二维斑点追踪成像(2D-STI)对脑梗死患者的左心室功能进行全面评估,探讨脑梗死对心脏功能的影响及其与脑梗死病情严重程度的潜在关联。方法:选取2021年11月—2023年11月海阳市人民医院收治的脑梗死患者150例为实验组,并选择无脑梗死病史的健康体检人员30例为对照组。实验组患者根据Adama分型,分为A组(腔隙性梗死组,50例)、B组(中等面积梗死组,40例)、C组(大面积梗死组,60例)。所有受试者均接受2D-STI检查,比较各组的左室整体纵向应变(GLS)和左室整体环向应变(GCS),评估左心室功能的差异。结果:与对照组比较,B组及C组左室舒张末期内径(LVEDD)、左心房前后径(LAD)、舒张早期前向血流峰值流速E峰/舒张早期二尖瓣瓣环侧壁侧及间隔侧的心肌位移峰值e’(E/e’)均增高,C组左室射血分数(LVEF)、二尖瓣口舒张早期前向血流峰值流速E峰/舒张晚期峰值流速A峰(E/A)减低,差异有统计学意义(P<0.05);与A组相比,C组LVEDd、LAD、E/e’增高,LVEF、E/A减低,差异有统计学意义(P<0.05);与B组相比,C组LVEF、E/A减低(P<0.05)。与对照组比较,实验组各亚组的GLS均减低,B组及C组GCS减低(P<0.05);与A组相比,B组、C组GCS、GLS均减低,差异有统计学意义(P<0.05);与B组相比,C组GCS、GLS均减低,差异有统计学意义(P<0.05)。结论:2D-STI能够有效评价不同程度脑梗死患者的左心室功能,且脑梗死患者的左心室功能存在不同程度的损害,随着梗死程度的加重,左心室收缩功能逐渐减弱。Objective To comprehensively assess left ventricular function in patients with cerebral infarction using two-dimensional speckle tracking imaging(2D-STI)and explore the impact of cerebral infarction on cardiac function and its potential correlation with the severity of cerebral infarction.Methods A total of 150 patients with cerebral infarction admitted to Haiyang People's Hospital from November 2021 to November 2023 were enrolled as the experimental group,and 30 healthy individuals with no history of cerebral infarction were selected as the control group.The experimental group was further divided into Group A(lacunar infarction,n=50),Group B(moderate-sized infarction,n=40),and Group C(large-sized infarction,n=60)according to the Adama classification.All subjects underwent 2D-STI examination to compare global longitudinal strain(GLS)and global circumferential strain(GCS)of the left ventricular among groups and assess differences in left ventricular function.Results Compared with the control group,Groups B and C showed increased left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD),and the ratio of early diastolic mitral inflow velocity(E)to early diastolic mitral annular velocity(e')(E/e'),while Group C exhibited decreased left ventricular ejection fraction(LVEF)and the ratio of E to late diastolic mitral inflow velocity(A)(E/A),with statistically significant differences(P<0.05).Compared with Group A,Group C had increased LVEDD,LAD,and E/e'and decreased LVEF and E/A,with statistically significant differences(P<0.05).Compared with Group B,Group C showed decreased LVEF and E/A(P<0.05).Compared with the control group,all subgroups in the experimental group had decreased GLS,and Groups B and C had decreased GCS(P<0.05).Compared with Group A,Groups B and C had decreased GCS and GLS,with statistically significant differences(P<0.05).Compared with Group B,Group C had decreased GCS and GLS,with statistically significant differences(P<0.05).Conclusion 2D-STI can effectively evaluate left ventricular fun
