补气升提法联合盆底生物反馈电刺激治疗轻中度压力性尿失禁的临床研究

摘　　要：目的观察补气升提法联合盆底生物反馈电刺激治疗轻中度压力性尿失禁的临床效果。方法将70例轻中度压力性尿失禁患者按照随机数字表法分为2组,对照组35例采用盆底生物反馈电刺激治疗,治疗组35例在对照组基础上采用补气升提法治疗。观察治疗前后尿失禁严重度索引(ISI)量表、尿失禁影响问卷简表(ⅡQ-7)、尿动力学检查(最大尿流率、残余尿量、膀胱最大容量)、盆底肌肉功能(Ⅰ类肌纤维肌电压、Ⅱ类肌纤维肌电压)、中医证候评分指标,并统计临床疗效。结果治疗组总有效率91.43%(32/35),对照组总有效率74.29%(26/35),治疗组临床疗效优于对照组(P<0.05)。2组治疗后ISI量表(漏尿频率、漏尿量)、ⅡQ-7量表评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后最大尿流率、最大膀胱容量均较本组治疗前增高(P<0.05),残余尿量均降低(P<0.05),且治疗组治疗后最大尿流率、最大膀胱容量均高于对照组(P<0.05),残余尿量低于对照组(P<0.05)。2组治疗后盆底肌肌电压均较治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。2组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论补气升提法联合盆底电刺激治疗压力性尿失禁能进一步改善患者盆底功能,提高临床疗效,提高患者生活质量。Objective To explore the clinical effect of Buqi Shengti method combined with pelvic floor biofeedback electrical stimulation in the treatment of mild to moderate stress urinary incontinence(SUI).Methods Seventy patients with SUI were randomized 1∶1 to control group(pelvic floor biofeedback electrical stimulation)or treatment group(Buqi Shengti method combined with pelvic floor biofeedback electrical stimulation).The key observation was incontinence severity index(ISI)scale,Incontinence Impact Questionnaire short version(ⅡQ-7),urodynamic examination(maximum urinary flow rate,residual urine volume,maximum bladder volume),pelvic floor muscle function(classⅠmuscle fiber voltage,classⅡmuscle fiber voltage),traditional Chinese medicine(TCM)syndrome scores,and the clinical efficacy.Results The total effective rate in the treatment group was significantly better than that in the control group(91.43%[32/35]vs 74.29%[26/35],P<0.05).After treatment,ISI scores(frequency and volume of urine leakage)andⅡQ-7 scores in the two groups were significantly decreased(P<0.05),and the decrease in the treatment group was more significant(P<0.05).After treatment,significantly increased maximum urinary flow rate and maximum bladder capacity,and decreased residual urine volume in the two groups were found(all P<0.05),and the treatment group was superior to the control group for the above indexes(all P<0.05).After treatment,the pelvic floor muscle voltage in the two groups was significantly increased(P<0.05),and the increase in the treatment group was more pronounced(P<0.05).After treatment,the scores and total scores of TCM syndromes in the two groups were significantly lower(P<0.05),especially in the treatment group(P<0.05).Conclusion Buqi Shengti method combined with pelvic floor biofeedback electrical stimulation in the treatment of SUI can further improve the patient's pelvic floor function,improve the clinical efficacy,and raise their quality of life.
