健脾祛湿化浊汤治疗脾虚湿重型原发性高血压临床疗效观察

摘　　要：目的观察健脾祛湿化浊汤治疗脾虚湿重型原发性高血压的临床效果。方法将82例脾虚湿重型原发性高血压患者,将其随机分为治疗组(41例)和对照组(41例),对照组服用苯磺酸氨氯地平片,治疗组在对照组基础上服用健脾祛湿化浊汤,比较2组中医证候疗效和治疗前、治疗3个月后血压、血液流变学各指标、内皮素(ET)、一氧化氮(NO)、生活质量评分。结果治疗组总有效率90.24%(37/41),对照组总有效率65.85%(27/41),治疗组中医证候疗效优于对照组(P<0.05)。2组治疗后收缩压和舒张压均降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后血液流变学各指标均降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后NO均升高(P<0.05),ET均降低(P<0.05),且治疗组治疗后NO高于对照组(P<0.05),ET低于对照组(P<0.05)。2组治疗后生活质量各维度评分均降低(P<0.05),且治疗组均低于对照组(P<0.05)。结论对脾虚湿重型原发性高血压患者采用健脾祛湿化浊汤治疗,可有效降低患者血压水平,改善患者血液流变学及血管内皮功能,提高患者生活质量。Objective To analyze the clinical effect of Jianpi Qushi Huazhuo Decoction for essential hypertension(EH)of spleen deficiency and dampness.Methods Eighty-two patients with EH(spleen deficiency and dampness)were randomly assigned into treatment group(n=41)and control group(n=41).All patients were managed by amlodipine besylate tablets,and those in the treatment group additionally received Jianpi Qushi Huazhuo Decoction.The traditional Chinese medicine(TCM)syndrome scores,the changes in blood pressure,hemorheology indexes,endothelin(ET),nitric oxide(NO)and quality of life(QoL)score before treatment and 3 months after treatment were compared between groups.Results The overall effective rate of TCM syndrome in the treatment group was significantly better than that in the control group(90.24%[37/41]vs 65.85%[27/41],P<0.05).After treatment,systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the both groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).The hemorheology indexes in the both groups were significantly decreased(P<0.05),which were significantly lower in the treatment group than in the control group(P<0.05).After treatment,significantly increased NO and decreased ET were found in the both groups(all P<0.05),which were significantly pronounced in the treatment group(all P<0.05).The dimension scores of QoL in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more common than the control group(P<0.05).Conclusion Jianpi Qushi Huazhuo Decoction can effectively reduce blood pressure levels,improve hemorheology,vascular endothelial function,and increased the QoL in patients with EH of spleen deficiency and dampness.
