摘  要:目的对上海市金山区消除疟疾后输入性疟疾再传播风险监测结果进行分析,为制定防控策略提供参考依据。方法通过国家传染病报告信息管理系统和寄生虫病防治信息管理系统收集2016—2023年上海市金山区疟疾病例报告信息和流行病学个案调查表,同时收集2016—2023年上海市金山区人群疟原虫感染监测和传疟媒介监测数据,使用Excel 2010和SPSS 18.0软件进行统计分析。计数资料(率)差异的比较采用χ^(2)双侧检验,P<0.05为差异有统计学意义。结果2016—2023年上海市金山区共报告疟疾病例67例,除2022年外,其余年份均有病例报告。其中恶性疟62例(92.54%),卵形疟4例(5.97%),间日疟1例(1.49%)。所有病例均为境外输入病例,感染来源地主要为尼日利亚(16.42%,11/67)、刚果民主共和国(13.43%,9/67)和喀麦隆(10.45%,7/67)等非洲国家。病例报告时间无明显季节性分布;人群分布以男性青壮年为主,主要为外出务工回国人员;具有金山区户籍病例1例(1.49%),其余为上海市其他县区籍(19.40%,13/67)、外省市籍(73.13%,49/67)和外籍(5.97%,4/67)。病例均由被动监测发现,检测阳性率为1.71%(162/9491)。从发病到诊断时间间隔中位数为4 d,发病后3 d内诊断的病例占47.76%(32/67)。病例初次就诊单位主要为省级医疗机构,占61.19%(41/67);初次就诊诊断为疟疾的病例占68.66%(46/67)。所有病例均按照上海市“1-2-3+1”工作要求进行处置,规范处置率为100%。2016—2023年上海市金山区媒介监测显示中华按蚊为本区唯一传疟媒介。结论消除疟疾后,金山区输入性疟疾疫情引起本地再传播的风险依然存在,应继续加强对输入性疟疾的监测和管理,持续巩固消除疟疾成果。Objective To analyze the monitoring results of the re-transmission risk of imported malaria after elimination in Jinshan District,Shanghai,China,so as to provide a reference basis for the development of prevention and control strategies.Methods Information on malaria case reports and epidemiologic case questionnaires in Jinshan District,Shanghai,from 2016 to 2023 were collected through National Information System for Infectious Disease surveillance and National Information System for Parasitic Diseases Control and Prevention,while data on surveillance of Plasmodium infection in the population of Jinshan District,Shanghai,and surveillance of malaria-transmitting vectors were collected in 2016-2023,and analyzed using Excel 2010 and SPSS 18.0 software for statistical analysis.The counting data(rates)were compared using the Chi-square test,P<0.05 indicated a statistically significant difference.Results A total of 67 malaria cases were reported in Jinshan District,Shanghai from 2016 to 2023,with cases reported in all years except 2022,including 62 cases(92.54%)infected with Plasmodium falciparum,4 cases(5.97%)infected with P.ovale,and one case(1.49%)infected with P.vivax.All cases were imported mainly from African countries such as Nigeria(16.42%,11/67),Democratic Republic of Congo(13.43%,9/67)and Cameroon(10.45%,7/67).There was no obvious seasonal distribution of the cases.Reported cases are predominantly male young adults,and mainly returnees who have gone abroad to work.There was only one case with Jinshan district household registration(1.49%),and the rest were from other counties and districts in Shanghai(19.40%,13/67),foreign provinces and cities(73.13%,49/67),and foreigners(5.97%,4/67).All cases were detected by passive surveillance with a positive test rate of 1.71%(162/9491).The median time from the illness onset to confirmed diagnosis was 4 d,and 47.76%(32/67)of cases were diagnosed within 3 d after the illness onset.The institutions where cases were first seen were mainly provincial medical institutions,acc
