摘  要:目的:基于静息态功能MRI(rs-fMRI)探讨亚急性颈痛患者的中枢发病机制。方法:于2022年9月-2023年8月招募32例亚急性颈痛患者和31例年龄、性别相匹配的健康志愿者(正常对照组)。使用3.0T磁共振仪对每例被试行常规序列(结构像)和rs-fMRI扫描。采用基于体素的形态学测量方法(VBM)表征两组受试者灰质体积的差异,采用局部一致性(ReHo)分析方法评价受试者大脑自发神经活动的特点。结果:与正常对照组比较,亚急性颈痛患者的右侧颞中回、左侧梭状回、左侧中央前回和右侧背外侧额上回的灰质体积减少(GRF校正,体素水平P<0.05,簇水平P<0.05);左侧小脑下部、左侧脑岛、右侧中央前回和左侧楔叶的ReHo值增加,左侧缘上回的ReHo值减小(GRF校正,体素水平P<0.05,簇水平P<0.05)。结论:亚急性颈痛患者的大脑内同时存在上、下行疼痛通路相关脑区结构和功能活动的异常。Objective:To investigate the central pathogenic mechanisms in patients with subacute neck pain using resting-state functional magnetic resonance imaging(rs-fMRI).Methods:From September 2022 to August 2023,32 individuals suffering from subacute neck pain and 31 healthy controls(HC group)with matched age and gender as the control group were recruited in the study.All subjects underwent conventional sequences(structural imaging)and rs-fMRI scanning at a 3.0T scanner.Then VBM method was used to find out the difference in gray matter volume between the two groups,and regional homogeneity(ReHo)analysis was performed to evaluate the spontaneous neural activity characteristics of brain.Results:Compared with HC group,neck pain group exhibited decreased gray matter volume in the right middle temporal gyrus,left fusiform gyrus,left precentral gyrus,and right dorsolateral superior frontal gyrus(GRF correction,voxel level P<0.05,cluster level P<0.05);Additionally,they showed increased ReHo values in left inferior cerebellum,left insula,right precentral gyrus,and left precuneus,decreased ReHo values in left supramarginal gyrus(GRF correction,voxel level P<0.05,cluster level P<0.05).Conclusion:Patients with subacute neck pain exhibit structural and functional anomalies in brain regions associated with both ascending and descending pain pathways.
