医防融合管理模式对老年慢性病患者血糖血脂水平及疾病认知程度的影响

摘　　要：目的 探讨医防融合管理模式在老年慢性病患者中的应用效果。方法 选择我院2022年1月至2023年12月收治的100例老年慢性病患者,按随机数字表法分为2组。对照组(50例)实施常规管理,观察组(50例)在对照组基础上采取医防融合管理模式,2组均持续随访6个月。对比2组患者血压、血糖、血脂水平,疾病认知程度情况,生活质量。结果 观察组管理后血压、血糖、血脂指标水平均低于对照组,差异有统计学意义(P<0.05);观察组管理后疾病认知程度评分均高于对照组,差异有统计学意义(P<0.05);观察组管理后生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论 给予老年慢性病患者医防融合管理模式进行干预,可提升其疾病认知程度,促使其积极参与到自我管理中,进而利于改善血压、血糖及血脂指标水平,提升生活质量。Objective To explore the application effect of the fusion management mode in elderly patients with chronic diseases.Methods A total of 100 elderly patients with chronic diseases admitted to our hospital from January 2022 to December 2023 were selected and divided into two groups according to the random number table method.The control group(50 cases)implemented routine management,and the observation group(50 cases)adopted the management mode of medical and prevention integration on the basis of the control group,and both groups were continuously followed up for 6 months.The blood pressure,blood glucose,blood lipid levels,disease awareness,and quality of life between the two groups were compared.Results The levels of blood pressure,blood glucose,and blood lipid indicators in the observation group were lower than those in the control group after management,and the differences were statistically significant(P<0.05).The disease cognition scores of the observation group were higher than those of the control group after management,and the difference was statistically significant(P<0.05).The quality of life score of the observation group after management was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Intervening in the integrated management model of medical prevention for elderly patients with chronic diseases can enhance their disease awareness,encourage their active participation in self-management,and thereby improve their blood pressure,blood glucose,and blood lipid levels,and enhance their quality of life.
