摘 要:瘢痕包括生理性和病理性两大类,其中病理性瘢痕又分为萎缩性、增生性瘢痕和瘢痕疙瘩。病理性瘢痕不仅会影响患者的外表美观,还可能导致疼痛、瘙痒等不适症状。光电技术如染料激光、强脉冲光、Nd:YAG激光、剥脱性点阵激光、非剥脱性点阵激光、微针射频和微等离子体射频等目前已被广泛应用于各型病理性瘢痕的治疗。不同技术治疗瘢痕的作用和机制不尽相同,主要是通过靶向血管抑制血流、减少成纤维细胞过度增殖和促进胶原蛋白的重塑实现瘢痕外观的改善和症状的减轻。本文全面综述了光电技术在病理性瘢痕中的治疗机制、疗效、策略以及可能不良反应,旨在为临床病理性瘢痕的个体化、精准化光电治疗提供全面的、可靠的循证医学证据,也为今后瘢痕光电治疗的进一步探索和扩大应用指明方向。Scars can be categorized into physiological and pathological types,with the latter further classified into atrophic,hypertrophic,and keloid scars.These pathological scars not only impair the aesthetic appearance of patients but may also lead to discomforting symptoms such as pain and itching.To alleviate these symptoms,a range of photoelectric technologies,including dye lasers,intense pulsed light,Nd:YAG lasers,ablative fractional lasers,non-ablative fractional lasers,microneedle radiofrequency,and micro-plasma radiofrequency,have been extensively applied in different types of pathological scar.Different technologies have different effects and mechanisms in treating scars.They are mainly aimed at improving scar appearance and alleviating symptoms by inhibiting blood flow,reducing fibroblast overproliferation and promoting collagen remodeling.This article comprehensively reviews the treatment mechanism,efficacy,strategies and possible adverse reactions of photoelectric technologies in pathological scars.It aims to provide comprehensive and reliable evidence-based medical evidence for individualized and precise photoelectric treatment of clinical pathological scars,and also points out the direction for further exploration and expanded application of photoelectric treatment of scars in the future.
