摘  要:目的探讨腰椎间盘突出症(lumbar disc herniation,LDH)患者退变椎间盘髓核组织中沉默信息调节因子1(silent information regulator 1,SIRT1)、核转录因子-κB(nuclear factor-κB,NF-κB)的表达,并分析二者与LDH退变程度的相关性。方法前瞻性选择2022年5月至2024年2月接受手术治疗的124例的LDH患者作为研究对象。根据LDH患者腰椎退变程度分为轻度组(n=31)、中度组(n=50)、重度组(n=43),分析不同退变程度患者一般资料及椎间盘髓核组织中SIRT1、NF-κB表达水平。经Pearson双变量相关性检验LDH患者椎间盘髓核组织中SIRT1、NF-κB与Pfirrmann分级的相关性;采用有序回归分析检验椎间盘髓核组织中SIRT1、NF-κB对LDH退变程度的影响。结果随着Pfirrmann分级增加,患者椎间盘髓核组织中SIRT1表达降低,NF-κB表达升高(P<0.05)。经Pearson双变量相关性检验,结果显示,椎间盘髓核组织中SIRT1表达与Pfirrmann分级呈负相关(r=-0.437,P<0.001),NF-κB表达与Pfirrmann分级呈正相关(r=0.493,P<0.001)。经有序回归分析结果显示,病理分型游离型、年龄、体质指数、椎间盘髓核组织中NF-κB、SIRT1是LDH退变程度的独立影响因素(OR>1,P<0.05)。该回归模型-2对数似然χ^(2)=94.026(P<0.001),拟合优度χ^(2)=286.001(P=0.018),说明该模型拟合优度良好。结论LDH患者退变椎间盘组织中SIRT1表达降低,NF-κB表达升高,且二者表达水平与腰椎退变程度密切相关。Objective To investigate the expression of silent information regulator 1(SIRT1)and nuclear factor-kappa B(NF-κB)in the nucleus pulposus tissue of degenerated discs in patients with lumbar disc herniation(LDH)and analyze their correlation with the degree of degeneration.Methods A total of 124 LDH patients undergoing surgical treatment from May 2022 to February 2024 were prospectively selected.The patients were categorized into mild,moderate,and severe degeneration groups according to the Pfirrmann grading system.The levels of SIRT1 and NF-κB expression in the nucleus pulposus tissue were measured and compared across groups.Pearson′s correlation analysis was used to examine the relationship between SIRT1,NF-κB expression,and Pfirrmann grades.Ordinal regression analysis was applied to determine the impact of SIRT1 and NF-κB on the degree of disc degeneration.Results SIRT1 expression decreased,while NF-κB expression increased with higher Pfirrmann grades(P<0.05).Pearson′s correlation analysis indicated a negative correlation between SIRT1 and Pfirrmann grades(r=-0.437,P<0.001)and a positive correlation between NF-κB and Pfirrmann grades(r=0.493,P<0.001).Ordinal regression analysis identified disc pathology(sequestration type),age,body mass index(BMI),NF-κB expression,and SIRT1 expression as independent predictors of disc degeneration severity(OR>1,P<0.05).The model fit was validated with a-2 log-likelihoodχ^(2)=94.026(P<0.001)and a goodness-of-fit χ^(2)=286.001(P=0.018),indicating good model adequacy.Conclusion In LDH patients,the degeneration of lumbar discs is characterized by reduced SIRT1 expression and increased NF-κB expression.Both markers are closely associated with the degree of lumbar disc degeneration,providing potential targets for therapeutic intervention.
