摘  要:为了更准确快速地对犬骨关节疾病进行疼痛评价,本文收集84例患有骨关节疾病的患犬病例,采用熵权法对格拉斯哥综合疼痛量表简版(CMPS-SF)进行优化,使用优化后的CMPS-SF对87例骨关节疾病患犬病例进行临床回归验证。结果显示,CMPS-SF各评分项目的熵权由大到小依次为触诊反应>对疼痛部位的反应>叫声>行动>精神状态>肢体状态,其熵权比约为4∶4∶4∶3∶3∶2;修正后CMPS-SF总分最高为80分,最低为0分,并依据疼痛程度设定1~20分为一级疼痛,21~40分为二级疼痛,41~60分为三级疼痛,61~80分为四级疼痛。临床回归验证结果显示,骨折、骨关节炎、骨关节损伤和髋关节发育不良的疼痛评分分别为(53.85±13.79)、(33.63±10.95)、(29.28±7.44)和(29.64±7.45)分,骨折与后3种骨关节疾病疼痛强度差异显著(P<0.05),后3种骨关节疾病间疼痛强度差异不显著(P>0.05)。结果表明,优化后的CMPS-SF更加科学合理,可用于临床上犬骨关节疾病的快速、准确评价和诊断。To achieve more accurate and rapid pain assessment in canine osteoarticular diseases,this study analyzed 84 cases of canine osteoarticular disease using the entropy weight method to optimize the short-form Glasgow composite measures pain scale(CMPS-SF).The optimized CMPS-SF was subsequently validated through clinical regression analysis on 87 cases.The results indicated that the entropy weights of CMPS-SF scoring items ranked from highest to lowest as palpation reaction>response to the painful area>vocalization>action>mental state>limb state,with a weight ratio of approximately 4∶4∶4∶3∶3∶2.The revised CMPS-SF had a total score ranging from 0 to 80,categorized into four pain levels:Level 1(1-20 points),Level 2(21-40 points),Level 3(41-60 points),and Level 4(61-80 points).Clinical regression validation revealed significant differences in pain scores among fracture(53.85±13.79),osteoarthritis(33.63±10.95),joint injury(29.28±7.44),and hip dysplasia(29.64±7.45),with fractures showing significantly higher pain intensity(P<0.05).No significant differences were observed among the latter three conditions(P>0.05).The findings demonstrate that the optimized CMPS-SF is more scientifically robust and suitable for rapid and accurate evaluation and diagnosis of canine osteoarticular diseases in clinical practice.
