腮腺肿瘤术后集束化护理策略实施及效果评价

摘　　要：目的观察腮腺肿瘤术后集束化护理策略实施效果。方法68例腮腺肿瘤患者随机分为对照组和观察组,各34例。对照组实施常规护理,观察组在对照组基础上实施集束化护理策略。评估两组干预后面神经功能恢复情况、焦虑抑郁状态、生活质量和咀嚼功能,比较两组护理满意度和并发症发生情况。结果观察组术后6个月House-Brackmann面神经功能分级标准(H-B)评分低于对照组(P<0.05)。干预后,两组患者焦虑自评量表(SAS)和抑郁自评量表(SDS)评分均降低,且观察组评分改善优于对照组(P<0.05)。干预后,两组生活质量升高,且观察组简明健康状况调查问卷各维度评分优于对照组(P<0.05)。观察组术后咀嚼效率显著高于对照组(P<0.05);观察组患者护理服务满意度对照组(P<0.05);观察组未发生并发症,对照组发生感染1(2.94%)例、暂时性面瘫3(8.82%)例、涎瘘1(2.94%)例。结论集束化护理策略的实施有助于促进腮腺肿瘤术后创面的愈合及面神经功能的恢复,降低并发症风险,改善患者不良心理状态,提高生活质量。Objective To investigate the effect of bundled nursing strategy in the nursing of postoperative patients with tumors of the parotid gland.Methods A total of 68 patients with pa⁃rotid gland tumors admitted to our hospital from January 2022 to December 2023 were randomly divided into two groups(control group and observation group),with 34 patients in each group.The control group received routine care,while the observation group received bundled care.The facial nerve function by House-Brackmann facial grading systems,negative emotion by Self-rat⁃ing Anxiety Scale and Self-rating Depressing Scale,quality of life scores by short-form question⁃naire,masticatory function,incidence of postoperative complications and patient satisfaction with nursing service were measure and compared between two groups.Results The score of House-Brackmann facial grading systems in the observation group was lower than that in the control group at 6 months after surgery(P<0.05).Scores of SAS and SDS decreased in both control group and observation group after intervention,and scores of SAS and SDS in the observation group were lower than those in the control group(P<0.05).Higher scores of SF-36 subscales were associated with bundled care in the observation group(P<0.05).Patients in the observa⁃tion group had a better outcome of masticatory efficiency assessment and higher level of nursing satisfaction compared with those in the control group(P<0.05).There was a case of infection,3 cases of Temporary facial paralysis and 1 case of salivary fistula observed in the control group.Conclusion Bundled nursing could effectively accelerate the postoperative wound healing and fa⁃cial nerve function recovery,reduced the occurrence of postoperative complications,and improve patient satisfaction and quality of life.
