复方黄柏液涂剂预防下颌近中阻生第三磨牙术后并发症的临床效果观察

摘　　要：目的:研究复方黄柏液涂剂作为拔牙术后预防用药,能否有效降低拔牙术后疼痛、肿胀、张口受限等并发症的严重程度。方法:收集广州医科大学附属口腔医院口腔颌面外科2023年1月-2023年12月期间,近中阻生下颌第三磨牙拔除术的病例。随机分成两组,试验组采用复方黄柏液涂剂进行术中拔牙窝冲洗和术后含漱、对照组使用生理盐水进行术中拔牙窝冲洗和术后含漱。主要评价指标是患者术后1周内最大疼痛程度(视觉模拟量表评分),次要评价指标是拔牙术后相关生活质量评分。结果:80例患者纳入研究,其中男性17例、女性63例,平均年龄(27.5±4.7)岁。复方黄柏涂剂组45例、生理盐水组35例。患者术后1周内最大疼痛程度,复方黄柏涂剂组(3.35±2.06)分:较生理盐水组(4.63±2.12)分:低,P=0.008。拔牙术后疼痛天数,复方黄柏涂剂组(2.49±1.47)d:较生理盐水组(3.37±1.85)d:短,P=0.024。拔牙术后肿胀天数,复方黄柏涂剂组(2.38±1.64)d:较生理盐水组(3.46±1.54)d:短,P=0.004。张口受限天数,复方黄柏涂剂组(2.84±1.1.55)d:较生理盐水组(3.40±1.56)d:短,P=0.117。影响进食天数,复方黄柏涂剂组(2.91±1.53)d:较生理盐水组(3.57±1.63)d:少,P=0.067。影响正常工作天数,复方黄柏涂剂组(1.13±1.36)d:较生理盐水组(2.09±1.85)d:少,P=0.013。影响正常生活天数,复方黄柏涂剂组(1.62±1.64)d:较生理盐水组(2.57±2.13)短,P=0.027。结论:复方黄柏液涂剂可有效预防并降低下颌第三磨牙近中阻生患者拔牙术后疼痛程度及对正常生活及工作的影响,在一定程度上提高了拔牙术后舒适度。Objective:To identify whether compound Huangbai liquid as a preventive medication after tooth extraction can effectively reduce the severity of postoperative complications such as pain,swelling,and restricted mouth opening.Methods:The cases of extraction of mesio-impacted mandibular third molars were collected in the Department of Oral Surgery,Affiliated Stomatological Hospital of Guangzhou Medical University,from January 2023 to December 2023.The patients were randomly divided into two groups.The treatment group was treated with compound Huangbai liquid for intraoperative tooth extraction socket irrigation and postoperative gargle,while the control group was treated with physiological saline for intraoperative tooth extraction socket irrigation and postoperative gargle.The main evaluation index was the maximum pain degree(visual analogue scale score)within one week after surgery,and the secondary evaluation index was the postoperative quality of life score.Results:A total of 80 patients were enrolled in the present study,including 17 men and 63 women,average age of(27.5±4.7)years.There were 45 patients in treatment group and 35 patients in control group.The maximum degree of pain within 1 week followed surgery in the treatment group(3.35±2.06)was lower than that in the control group(4.63±2.12),P=0.008.The duration of pain after tooth extraction in the treatment group,(2.49±1.47)days,was shorter than that in the control group,(3.37±1.85)days,P=0.024.The swelling days after tooth extraction in the treatment group,(2.38±1.64)days,were shorter than those in the control group,(3.46±1.54)days,P=0.004.The number of days with limited mouth opening in the treatment group,(2.84±1.1.55)days,was shorter than that in the control group,(3.40±1.56)days,P=0.117.The number of days affecting eating in the treatment group,(2.91±1.53)days,was less than that in the control group,(3.57±1.63)days,P=0.067.The impact on normal work days in the experimental group,(1.13±1.36)days,was less than that in the control group,(2.09±
