基于属性层次模型的奥马环素临床应用合理性评价

摘　　要：目的:评价吉林省人民医院临床应用注射用甲苯磺酸奥马环素的合理性。方法:合理用药评价小组以奥马环素药品说明书、相关临床指南共识为依据,制定用药合理性评价标准;采用回顾性研究方法,基于属性层次模型(attribute hierarchical model,AHM),对我院2024年1月-2024年7月应用该药的90例病历进行评价分析。结果:我院该药的临床应用主要存在用药指征欠合理、首剂未予以负荷剂量、疗程较长等不合理问题。利用AHM法最终获得各病历评分(medical record score,MRS),MRS<60分的4例、60≤MRS<80分的8例、80≤MRS<90分的48例、90≤MRS≤100分的30例。结论:我院奥马环素临床使用合理性仍有待提高,需进一步采取有效措施进行针对性干预。AHM作为药物合理性评价新方法,具有科学合理、真实客观、简便实用的优势。Objective:To evaluate the clinical rationality for the use of omadacycline toluene sulfonate for injection at Jilin Provincial People's Hospital.Methods:The Rational Drug Use Evaluation Group has established the criteria for rational drug use based on a consensus of the omadacycline drug instructions and related clinical guidelines.A retrospective study was conducted utilizing an attribute hierarchy model(AHM)to evaluate and analyze the medical records of 90 patients who received this drug at our hospital from January 2024 to July 2024.Results:The main problems in the clinical application of this drug at our hospital included unreasonable indications,no loading dose in the first dose and a long course of treatment.Medical record score(MRS)was obtained by AHM method.There were 4 cases with MRS<60 points,8 cases with 60≤MRS<80 points,48 cases with 80≤MRS<90 points,and 30 cases with 90≤MRS<100 points.Conclusion:The rationality of clinical use of omadacycline at our hospital still requires enhancement,and further effective measures should be taken for targeted intervention.As a new method for drug rationality evaluation,AHM has the advantages of scientific rationality,real objective,simple and practical.
