湿热痹阻型强直性脊柱炎病因病机与治疗浅析

摘　　要：中医将强直性脊柱炎(ankylosing spondylitis,AS)与类风湿关节炎(rheumatoid arthritis,RA)、骨关节炎(osteoarthritis,OA)等同归为“痹证”范畴,强直性脊柱炎大多被称为“大偻”“骨痹”“龟背风”“竹节风”等。湿热痹阻型强直性脊柱炎的本质为本虚标实,肾虚为疾病之根本,湿热等外邪侵袭为该病诱因,病理产物瘀血贯穿疾病始终,阻滞经络气血运行,为该病病因病机。古代医家以清热利湿法治湿热痹为后世医家治疗湿热痹阻型强直性脊柱炎提供理论依据和经验,现代中医参考古代医家的理论经验并结合自己的临床实践,对清热利湿法治疗湿热痹阻型强直性脊柱炎有着深刻的见解和广泛应用。该病治疗时应采用清热利湿兼补肾之法,同时应注重化痰祛瘀。Traditional Chinese medicine(TCM)categorizes ankylosing spondylitis(AS),rheumatoid arthritis(RA),and osteoarthritis(OA)as'arthralgia syndrome'.AS is often referred to as'Dalei','Gubi','Guibeifeng','Zhujiefeng',and so on in TCM.The essence of AS with damp-heat obstruction type is deficiency in origin and excess in superficiality,with kidney deficiency as the root cause.External pathogens like damp-heat cause the disease,and the pathological product of blood stasis runs through the disease,blocking the circulation of meridian qi and blood-this is the etiology and pathogenesis of the disease.Ancient physicians used the method of clearing heat and removing dampness to treat damp-heat arthralgia,providing a theoretical basis and experience for later generations of physicians to treat damp-heat arthralgia type AS.Modern Chinese medicine refers to the theoretical experience of ancient physicians and combines their own clinical practice,offering profound insights and wide application in the treatment of damp-heat arthralgia type AS.In treatment,the method of clearing heat,removing dampness,and tonifying the kidney should be adopted,with attention to resolving phlegm and removing blood stasis.
