基于古中医“圆运动”理论探讨糖尿病胃轻瘫发病与治疗

摘　　要：糖尿病胃轻瘫是以血糖升高,饮食减少,餐时或餐前早饱感,上腹部胀闷不适,时有恶心呕吐,胃排空延迟但未见胃肠器质性病变作为临床常见糖尿病慢性并发症,其病位在脾胃,与肝胆相关,后期波及至肾。现代医学治疗虽能降低血糖,促胃肠蠕动,但仍有局限性,且易发生较多不良反应。中医“圆运动”理论认为,人体与自然界相通,人体气机无时无刻不在进行有规律的“圆运动”,即生理循环或病理循环。基于该理论治疗糖尿病胃轻瘫患者,从整体观念出发,通过调整“中轴”与“四维”运行状态,或升降脾胃,温运水湿,或升降肝胆,以行疏泄,或益肾填精,潜藏元真,进而纠正人体脏腑偏颇,使气机运动由偏复圆,使病理循环调整回生理循环,极大限度提高生活质量。Diabetic gastroparesis is a common chronic complication of diabetes,characterized by elevated blood sugar,reduced appetite,early satiation during or before meals,abdominal distension and discomfort,nausea and vomiting,delayed gastric emptying but no organic gastrointestinal diseases.Pathologically,the disease involves the spleen and stomach,is related to the liver and gallbladder,and affects the kidney in later stages.Although modern medical treatment can reduce blood sugar and promote gastrointestinal peristalsis,it still has limitations and can be prone to adverse reactions.The theory of"circle movement"in traditional Chinese medicine holds that the human body is connected with nature,and the human qi machine carries out regular"circle movement",i.e.,physiological or pathological cycles.Based on this theory,treatment of diabetic gastroparesis patients starts from an overall concept,adjusts the"axis"and"four-dimensional"operation state,aims to lift the spleen and stomach,warm water and dampness,or lift the liver and gallbladder,in order to relieve or tonify the kidney to fill essence,hide Yuan true,and then correct the bias of the viscera,so that the qi machine movement changes from partial circle to whole body,thus pathophysiological cycles are adjusted back to physiological,and greatly improve the quality of life.
