摘 要:目的评估硫辛酸治疗对精子DNA完整性的影响。方法使用“精子DNA碎片率”“Sperm DNA fragmentation rate”等中英文关键词,通过计算机检索Cochrane library、Pubmed、Embase等英文数据库及中国生物医学、中国知网、万方、维普等中文数据库,检索时间从建库至2024年3月31日。按照相应纳入、排除标准收集硫辛酸和安慰剂治疗对精子DNA完整性影响的随机对照试验(RCT),由两名独立研究者协同进行资料提取,纳入的文献质量评价应用Cochrane图书馆制定的风险偏倚评估工具进行,采用Meta分析的方法对硫辛酸治疗的效果进行评价。结果共纳入3篇RCT,试验组采用硫辛酸治疗,对照组采用安慰剂治疗。Meta分析结果表明:(1)与治疗前相比,硫辛酸能显著降低患者精子DNA碎片率[MD=-8.98,95%CI(-14.62,-3.34),P=0.002],但是对精子DNA片段化无显著改善(P>0.05);(2)与对照组安慰剂相比,硫辛酸对精子DNA碎片率及精子DNA片段化的改善均无显著差异(P>0.05)。结论硫辛酸可以有效降低精子DNA损伤,有利于增加精子DNA完整性;但与安慰剂比较,其改善精子DNA损伤和精子DNA片段化的效果无明显差异。Objective:To investigate the effect of lipoic acid treatment on sperm DNA integrity rate.Methods:Using keywords such as“sperm DNA fragmentation rate”in both Chinese and English,we searched English databases such as Cochrane library,Pubmed,Embase,as well as Chinese databases such as CNKI,Wanfang,and VIP through a computer.The search time was from the establishment of the database until March 31,2024.Randomized controlled trials(RCTs)on the effects of lipoic acid and placebo treatment on sperm DNA integrity were collected according to the corresponding inclusion and exclusion criteria.Two independent researchers collaborated to extract data,and the quality of the included literature was evaluated using the risk bias assessment tool developed by the Cochrane library.Meta analysis was used to evaluate the efficacy of lipoic acid treatment.Results:Three RCTs were included,with the experimental group treated with lipoic acid and the control group treated with placebo.The meta-analysis results showed that compared with before treatment,lipoic acid significantly reduced the sperm DNA fragmentation rate in patients[MD=-8.98,95%CI(-14.62,-3.34),P=0.002],but did not significantly improve sperm DNA fragmentation(P>0.05).Compared with the placebo group,there was no significant difference in the improvement of sperm DNA fragmentation rate and sperm DNA fragmentation by lipoic acid(P>0.05).Conclusions:Lipoic acid can effectively reduce sperm DNA damage and improve sperm DNA integrity.But compared with placebo,there was no significant difference in its effect on reducing sperm DNA damage and sperm DNA fragmentation.
