128层螺旋CT对颞骨骨折伴听小骨脱位的诊断价值

摘  要:目的研究128层螺旋CT对颞骨骨折伴听小骨脱位的诊断价值。方法选取本院于2022年1月至2023年5月收治的颞骨折患者30例,其中伴听小骨脱位患者20例,按照诊断方法进行分组,分别进行普通X线平片、常规轴位CT、128层螺旋CT诊断。结果128层螺旋CT诊断下骨折情况总检出率、听小骨脱位情况总检出率、诊断效能(准确度、灵敏度、特异度)、诊断图像质量评分(患者沟通、重照因素控制、图像质量要求)>常规轴位CT>普通X线平片,对比结果差异显著(P<0.05)。结论颞骨及耳部听小骨解剖结构复杂普通X线平片和常规轴位CT很难全面客观地显示骨折及脱位程度,128层螺旋CT以其快捷的扫描速度及完善的图像后处理功能对颞骨及听小骨的损伤诊断具有独特优势。Objective To study the diagnostic value of 128 slice spiral CT in temporal bone fracture with auditory ossicle dislocation.Method Thirty patients with temporal bone fractures admitted to our hospital from January 2022 to May 2023 were selected,including 20 patients with dislocation of the auditory ossicle.They were grouped according to the diagnostic method and diagnosed with conventional X-ray plain film,conventional axial CT,and 128 slice spiral CT,respectively.Result Under the diagnosis of 128 slice spiral CT,the total detection rate of fracture and auditory ossicle dislocation,diagnostic effcacy(accuracy,sensitivity,specificity),diagnostic image quality score(patient communication,re irradiation factor control,image quality requirements),conventional axial CT,and ordinary X-ray plain film were significantly different(P<0.05).Conclusion The anatomical structure of the temporal bone and auditory ossicle is complex,and it is difficult to comprehensively and objectively display the degree of fractures and dislocations using ordinary X-ray plain films and conventional axial CT.128 slice spiral CT has unique advantages in diagnosing temporal bone and auditory ossicle injuries with its fast scanning speed and complete image post-processing function.
