摘  要:目的分析病原菌检测在外耳道真菌病诊断中的临床价值。方法选择2022年7月到2023年7月期间因外耳道真菌病在本院耳鼻咽喉科就诊的100例患者作为研究对象,为其采集外耳道中的分泌物,进行菌株分离、培养、鉴定试验,观察真菌种类及分布情况。予以药敏试验,对比伊曲康唑、特比萘芬等药物的敏感性,以指明外耳道真菌病的治疗方向。结果真菌的菌种主要为土曲霉(58%)、黄曲霉(21%)、黑曲霉(11%)等曲霉菌,少数为念珠菌(6%)、其他真菌(4%)。大部分真菌对特比萘芬和伊曲康唑敏感性较高,对克霉唑和联苯苄唑敏感性较低。结论曲霉菌、念珠菌是导致外耳道真菌病的主要病原微生物,临床应根据药敏试验结果,为外耳道真菌病患者选择特比萘芬和伊曲康唑等敏感性较高的药物,以获得理想效果。Objective To analyze the clinical value of pathogenic bacteria detection in the diagnosis of external auditory canal mycosis.Methods From July 2022 to July 2023,100 patients with external auditory canal mycosis were selected as the research object.The secretions from external auditory canal were collected,and the strains were isolated,cultured and identified.The species and distribution of fungi were observed.Drug sensitivity test was performed to compare the sensitivity of itraconazole,terbinafine and other drugs to indicate the treatment direction of external auditory canal mycosis.Results The main species of fungi were Aspergillus terreus(58%),Aspergillus flavus(21%),Aspergillus niger(11%),and other fungi(4%)were Candida spp.(6%).Most fungi were highly sensitive to terbinafine and itraconazole,but less sensitive to clotrimazole and bifonazole.Conclusion Aspergillus and candida are the main pathogenic microorganisms causing external auditory canal mycosis.The clinical should select terbinafine and itraconazole with high sensitivity for patients with external auditory canal mycosis according to the results of drug sensitivity test in order to obtain ideal effect.
