环状软骨上喉部分切除术患者吞咽功能快速康复方案构建及可行性的研究

摘　　要：目的探究环状软骨上喉部分切除术患者吞咽功能快速康复方案构建及可行性。方法选取东南大学附属中大医院耳鼻咽喉头颈外科2023年2月至2023年10月收治的喉癌患者26例作为研究对象,随机分为两组,每组13例,对照组常规康复方案,试验组快速康复方案,对比患者康复指标及呛咳发生情况。结果试验组住院时间、胃管留置时间低于对照组,再插管例数低于对照组(P<0.05);试验组并发症低于对照组(P<0.05);试验组术后1周、2周SAS评分均低于对照组(P<0.05);试验组术后2周、3周误吸评分均低于对照组(P<0.05)。结论对于接受环状软骨上喉部分切除术治疗患者开展吞咽功能快速康复方案干预可有效降低患者误吸评分,缩短患者胃管留置时间,防止并发症的发生,促进患者康复。Objective To explore the construction and feasibility of rapid rehabilitation of swallowing function in patients with partial resection of superior cricoid larynx.Method Twenty-six patients with laryngeal cancer admitted to the Department of Otolaryngology,Head and Neck Surgery,Zhongda Hospital Affiliated to Southeast February 2023 to October 2023 were selected as the study objects and randomly divided into two groups,with 13 cases in each group.The control group received routine rehabilitation program and the experimental group received rapid rehabilitation program to compare the rehabilitation indicators and the occurrence of cough.Results The length of hospital stay and retention time of gastric tube in experimental group were lower than those in control group,and the number of re-intubation cases was lower than that in control group(P<0.05).The complication of experimental group was lower than that of control group(P<0.05).The SAS scores of experimental groups were lower than those of control group 1 and 2 weeks after operation(P<0.05).The aspiration scores of experimental groups were lower than those of control group at 2 and 3 weeks after operation(P<0.05).Conclusion For the patients who received partial laryngectomy superior cricoid cartilage,rapid rehabilitation of swallowing function intervention can effectively reduce the aspiration score,shorten the retention time of gastric tube,reduce the occurrence of complications,and promote the recovery of patients.
