摘 要:目的探索急性心肌梗死(AMI)患者早期免疫细胞图谱及其变化特征。方法研究收集了19例AMI患者(男性11例,女性8例)和10例稳定型心绞痛(SAP)患者(男女各5例)的外周血标本,分离单个核细胞,应用不同金属耦联的42种细胞表面标志物进行标记,通过单细胞质谱流式技术进行精细分群和比较分析。结果通过单细胞质谱流式技术,将免疫细胞精细分为8个大亚群和53个小亚群(C1~C53)。在大亚群的层面,与同性别SAP患者相比较,男性AMI患者的单核细胞比例显著升高、CD4^(+)T细胞比例显著降低;女性AMI患者单核细胞、B细胞和浆细胞样树突状细胞比例显著升高,CD4^(+)T细胞和CD8^(+)T细胞比例则显著降低。在小亚群层面,男性AMI患者单核细胞C19、CD8^(+)T细胞C36和C37、CD4^(+)T细胞C45的比例,与男性SAP患者相比较存在显著差异。在女性患者中,AMI患者单核细胞C18、B细胞C27、CD8^(+)T细胞C36和C39、CD4^(+)T细胞C45和C52等8个小亚群的比例,与SAP患者存在显著差异。结论本研究描绘了不同性别AMI患者循环免疫细胞亚群的变化,为探索AMI免疫反应以及性别差异提供了新的研究思路。Objective To explore the early immune cell atlas and its changing characteristics in patients with acute myocardial infarction(AMI).Methods This study collected peripheral blood samples from 19 patients with AMI(11 males and 8 females)and 10 patients with stable angina pectoris(SAP)(5 males and 5 females).Mononuclear cells were isolated and labeled with 42 different metal labeled cell surface markers.Fine clustering and comparative analysis were performed using single-cell mass spectrometry flow cytometry.Results Through single-cell mass spectrometry flow cytometry,this study finely divided immune cells into 8 large subgroups and 53 small subgroups(C1~C53).At the level of large subgroups,compared to same-sex SAP patients,the proportion of monocytes in male AMI patients significantly increased and the proportion of CD4^(+)T cells significantly decreased.The proportion of monocytes,B cells and plasmacytoid dendnitic cells in female AMI patients significantly increased,the proportion of CD4^(+)T cells and CD8^(+)T cells significantly decreased.At the small subgroup level,the proportions of monocyte C19,CD8^(+)T cell C36 and C37,and CD4^(+)T cell C45 were significantly different in male AMI patients compared with SAP patients.In female patients,the proportions of 8 small subgroups,including monocyte C18,B cell C27,CD8^(+)T cell C36 and C39,CD4^(+)T cell C45 and C52,in AMI patients were significantly different from those in SAP patients.Conclusion This study describes the changes in circulating immune cell subsets in patients with AMI of different genders,providing new research ideas for exploring immune response and gender differences of AMI.
