摘  要:目的:采用网状Meta分析方法评价不同非药物干预改善系统性红斑狼疮病人疲劳的效果,以期为改善系统性红斑狼疮病人疲劳状况护理实践提供参考依据。方法:计算机检索中国知网、维普数据库、万方数据库和中国生物医学文献数据库、PubMed、the Cochrane Library、ScienceDirect、Medline、Embase,检索时限均为从建库至2024年6月,采用Stata 17.0软件进行网状Meta分析。结果:共纳入17项随机对照研究(RCT),包含7种非药物干预方法。网状Meta分析结果显示,运动干预、营养干预、针刺疗法对改善系统性红斑狼疮病人疲劳效果均优于常规护理(P<0.05);在纳入的7种非药物干预措施中,运动干预优于除营养干预外的其他干预措施(P<0.05),营养干预优于健康教育、生活方式干预、心理干预(P<0.05)。累积排序概率图下面积排序:运动干预>营养干预>针刺疗法>经皮迷走神经刺激>健康教育>生活方式干预>心理干预>常规护理。结论:根据网状Meta分析结果,在无运动禁忌证的情况下,推荐临床首选运动干预为缓解系统性红斑狼疮病人疲劳的非药物干预措施,其次为营养干预。Objective:Using the network Meta-analysis method to evaluate the effect of different non-pharmacological interventions on improving fatigue in patients with systemic lupus erythematosus,in order to provide reference for nursing practice to improve fatigue status in patients with systemic lupus erythematosus.Methods:Computer searches of PubMed,the Cochrane Library,ScienceDirect,Medline,Embase,CNKI,VIP Database,Wanfang Database,and China Biomedical Literature Database were conducted from database establishment to June 2024.Stata17.0 was used for a network Meta-analysis.Results:A total of 17 RCT studies were included,including 7 non-pharmacological intervention methods.The results of the network Meta-analysis showed that exercise intervention,nutritional intervention,and acupuncture therapy had better effects on improving fatigue in patients with systemic lupus erythematosus than conventional nursing(P<0.05).Among the 7 non-pharmacological interventions included,exercise intervention was superior to other interventions except for nutritional intervention(P<0.05),while nutritional intervention was superior to health education,lifestyle intervention,and psychological intervention(P<0.05).Area ranking under the cumulative ranking probability chart:exercise intervention>nutritional intervention>acupuncture therapy>percutaneous vagus nerve stimulation>health education>lifestyle intervention>psychological intervention>routine nursing.Conclusions:Based on the results of the network meta-analysis and probability ranking,in the absence of contraindications to exercise,it is recommended that the clinical preferred exercise intervention be a non-pharmacological intervention to alleviate fatigue in patients with systemic lupus erythematosus,followed by nutritional intervention or acupuncture therapy.
