体检人群甲状腺结节与甲状腺功能及代谢指标的临床分析

摘　　要：为了解当地人群甲状腺结节发生率及其影响因素,选取闽东地区某三甲医院2022年7月至2023年6月的体检人群750例,统计不同人群的甲状腺结节发生情况,比较甲状腺结节患者与非甲状腺结节受检者的甲状腺功能及代谢指标变化,并探析其影响因素。结果显示,当地体检人群的甲状腺结节检出率为64.67%,不同代谢异常人群的甲状腺结节检出率由高到低依次为糖代谢异常人群(71.74%)、超重/肥胖人群(70.93%)、高血压人群(69.66%)、血脂异常人群(66.09%)、高尿酸人群(65.45%)及甲功异常人群(61.18%);女性人群甲状腺结节高发显著,尤其是高级别甲状腺结节;随着年龄增长及代谢异常出现(血糖、血脂、血压、肥胖、甲功)发生高级别甲状腺结节概率总体增高;性别、年龄、体重指数、收缩压、低密度脂蛋白及过氧化物酶抗体为甲状腺结节的影响因素。The objective of this study was to learn the incidence of thyroid nodules in local population and its influencing factors.The subjects of this study were 750 patients who underwent physical examination from July 2022 to June 2023 in a top three hospital in East Fujian province.The incidence of thyroid nodules was ana⁃lyzed,and the changes of thyroid function and metabolism indexes were compared between patients with thy⁃roid nodules and non-thyroid nodules,and the influencing factors were analyzed.The results show that the de⁃tection rate of thyroid nodules in the local population is 64.67%.The detection rates of thyroid nodules ranging from high to low are as follow:abnormal glucose metabolism(71.74%),overweight/obese(70.93%),hyperten⁃sion(69.66%),dyslipidemia(66.09%),high uric acid(65.45%)and abnormal thyroid function(61.18%),The incidence of thyroid nodules is significantly higher in female population,especially high-grade thyroid nod⁃ules.The probability of high-grade thyroid nodules increases with age and metabolic abnormalities(blood glu⁃cose,blood lipids,blood pressure,obesity,and thyroid function).Gender,age,body mass index,systolic blood pressure,low density lipoprotein and peroxidase antibody are all factors affecting thyroid nodules.
