生活自理能力水平在老年心衰患者衰弱评估中的价值

摘　　要：目的调查老年心力衰竭(HF)患者衰弱及生活自理能力现状,探讨生活自理能力水平在衰弱评估中的价值。方法选取2023年10月至2024年1月本院心内科收治的127例老年HF患者,根据衰弱程度将其分为无衰弱组、衰弱前期组、衰弱组。比较三组的一般资料、实验室指标及心脏彩超指标;分析老年HF患者衰弱发生情况的影响因素。结果127例老年HF患者中,11例(8.66%)无衰弱,49例(38.58%)为衰弱前期,67例(52.76%)衰弱。三组患者的饮酒史、体育运动、生活自理能力、夜间睡眠时长比较,差异具有统计学意义(P<0.05)。生活自理能力是衰弱的独立影响因素(P<0.05)。结论老年HF患者的衰弱患病率高;在临床工作中,尤其针对Barthel指数≤60分的老年HF患者,生活自理能力水平可一定程度上为衰弱评估提供参考。Objective To investigate the current status of frailty and self-care ability in elderly patients with heart failure(HF),and to explore the value of self-care ability level in the assessment of frailty.Methods A total of 127 elderly patients with HF admitted in the cardiology department of our hospital from October 2023 to January 2024 were selected.According to the degree of frailty,the patients were divided into non-frailty group,pre-frailty group and frailty group.The general data,laboratory indexes and echocardiography indexes of the three groups were compared;the influencing factors of frailty in elderly patients with HF was analyzed.Results Among 127 elderly patients with HF,11 cases(8.66%)had no frailty,49 cases(38.58%)had pre-frailty,and 67 cases(52.76%)had frailty.There were statistically significant differences in drinking history,physical exercise,self-care ability and nighttime sleep duration among the three groups(P<0.05).Self-care ability was an independent influencing factor of frailty(P<0.05).Conclusion The prevalence of frailty in elderly patients with HF is high;in clinical work,especially for elderly HF patients with Barthel index≤60 points,the self-care ability level can provide a reference for frailty assessment to a certain extent.
