摘　　要：目的探讨插入式灌肠引流装置在先天性巨结肠患儿术前准备中的应用效果。方法随机将2020年1月至2022年1月于我院就诊并行手术治疗的64例先天性巨结肠患儿分为对照组和研究组,各32例。对照组使用传统回流灌肠方法进行术前肠道准备,研究组使用插入式灌肠引流装置进行回流灌肠。比较两组的术前灌肠次数、单次灌肠时间、肠道清洁率、灌肠液出入量准确率、腹围减小率、床单元清洁率、舒适度和并发症发生情况。结果研究组的术前灌肠次数明显少于对照组,单次灌肠时间明显短于对照组,肠道清洁率、灌肠液出入量准确率、腹围减小率、床单元清洁率、灌肠舒适度优良率明显高于对照组(P<0.05)。两组的灌肠相关并发症总发生率无显著差异(P>0.05)。结论先天性巨结肠患儿术前采用插入式灌肠引流装置进行回流灌肠可明显减少灌肠次数和单次灌肠时间,提高肠道清洁度和床单元清洁度,且患儿舒适度及安全性更高。Objective To explore the application effect of insertion type enema drainage device in preoperative preparation of children with Hirschsprung's disease.Methods A total of 64 children with Hirschsprung's disease underwent surgical treatment in hospital from January 2020 to January 2022 were randomly divided into control group and study group,with 32 cases in each group.The control group used the traditional reflux enema method for preoperative bowel preparation,and the study group used the insertion type enema drainage device for reflux enema.The preoperative enema times,single enema time,intestinal cleaning rate,accuracy of enema fluid intake and output,abdominal circumference reduction rate,bed unit cleaning rate,comfort and complications were compared between the two groups.Results The preoperative enema times in the study group was significantly less than that in the control group,the single enema time was significantly shorter than that in the control group,and the intestinal cleaning rate,accuracy of enema fluid intake and output,abdominal circumference reduction rate,bed unit cleaning rate,and excellent and good rate of enema comfort were significantly higher than those in the control group(P<0.05).There was no significant difference in the total incidence of enema-related complications between the two groups(P>0.05).Conclusion Preoperative reflux enema with insertion type enema drainage device in children with Hirschsprung's disease can significantly reduce the enema times and single enema time,and improve intestinal cleanliness and bed unit cleanliness,with higher comfort and safety of children.
