摘　　要：目的探讨司库奇尤单抗治疗中重度斑块状银屑病合并慢性乙肝的效果。方法选取2021年3月至8月就诊于我院皮肤科的10例中重度斑块状银屑病合并慢性乙肝患者作为研究对象,应用司库奇尤单抗治疗。评估并记录患者的近、远期疗效,分析用药安全性。结果治疗第4周,10例患者的皮损均有不同程度的改善,9例患者皮损改善达标,1例患者银屑病皮损面积和严重程度指数(PASI)50;治疗第8周,所有患者皮损改善均≥PASI 75,均达标;治疗第12周,临床疗效维持较好,未见皮损反复或加重。10例患者均完成为期1年的治疗和随访,其中1例出现皮疹轻度反复,1例出现轻度脱发,1例出现轻度毛囊炎症状,给予相应对症治疗后病情得以控制,均未发生严重不良反应。结论司库奇尤单抗治疗中重度斑块状银屑病合并慢性乙肝患者的效果较好,不良反应少,安全性较高,值得临床应用。Objective To explore the effect of secukinumab in the treatment of moderate to severe plaque psoriasis with chronic hepatitis B.Methods From March to August 2021,10 patients with moderate to severe plaque psoriasis with chronic hepatitis B who were treated in the dermatology department of our hospital were selected as the research objects,and treated with secukinumab.The short-term and long-term efficacy of the patients was evaluated and recorded,and the safety of medication was analyzed.Results At the 4th week of treatment,the skin lesions of 10 patients were improved to varying degrees,9 patients reached the standard,and 1 patient had the Psoriasis Area and Severity Index Score(PASI)50;at the 8th week of treatment,the improvement of skin lesions in all patients was≥PASI 75,and all patients reached the standard;at the 12th week of treatment,the clinical efficacy was maintained well,and no recurrence or aggravation of skin lesions was observed.All 10 patients completed a one-year treatment and follow-up,among them,1 case had mild rash recurrence,1 case had mild alopecia,and 1 case had mild folliculitis symptoms;after corresponding symptomatic treatment,the condition was controlled,and no serious adverse reactions occurred.Conclusion Secukinumab is effective in the treatment of patients with moderate to severe plaque psoriasis complicated with chronic hepatitis B,with fewer adverse reactions and higher safety,which is worthy of clinical application.
