血清Caspase-1、DKK-1、IL-2Rα水平与系统性红斑狼疮患者病情程度的相关性分析及临床价值

摘　　要：目的探讨系统性红斑狼疮(SLE)患者血清半胱氨酰天冬氨酸特异性蛋白酶-1(Caspase-1)、Dickkopf-1(DKK-1)、白细胞介素-2受体α(IL-2Rα)水平,并分析其与病情程度相关性及其对肾功能损害的评估价值。方法回顾性选取收治的SLE患者114例作为研究组,另选取同期健康志愿者38例作为对照组。比较两组、研究组不同病情程度者血清Caspase-1、DKK-1、IL-2Rα水平。分析血清Caspase-1、DKK-1、IL-2Rα与临床指标[白细胞计数、血红蛋白、淋巴细胞计数、血小板、红细胞沉降率、血肌酐、24 h尿蛋白、抗双联DNA抗体(dsDNA)、补体C3、补体C4]、病情程度相关性。依据肾功能损害程度将研究组分为肾功能正常58例、轻度异常31例、中重度异常25例,比较其血清Caspase-1、DKK-1、IL-2Rα水平,并分析其对肾功能损害程度的评估价值。结果研究组血清Caspase-1水平低于对照组,血清DKK-1、IL-2Rα水平高于对照组(P<0.05);随着病情程度、肾功能损害程度加重,血清Caspase-1水平呈下降趋势,血清DKK-1、IL-2Rα水平呈上升趋势(P<0.05);血清Caspase-1与白细胞计数、淋巴细胞计数、补体C3、补体C4呈正相关,而与红细胞沉降率、抗dsDNA抗体、病情程度呈负相关(P<0.05);血清DKK-1、IL-2Rα与白细胞计数、淋巴细胞计数、补体C3、补体C4呈负相关,而与红细胞沉降率、抗dsDNA抗体、病情程度呈正相关(P<0.05);血清Caspase-1、DKK-1、IL-2Rα联合鉴别评估肾功能损害程度的AUC大于单项指标评估(P<0.05)。结论SLE患者血清Caspase-1水平降低,血清DKK-1、IL-2Rα水平升高,且与病情程度密切相关,联合检测其水平对肾功能损害程度具有一定评估价值。Objective To investigate the serum levels of Caspase-1(Caspase-1),Dickkopf-1(DKK-1)and interleukin-2 receptorα(IL-2Rα)in patients with systemic lupus erythematosus(SLE),and to analyze their correlation with the severity of the disease and their evaluation value in renal function impairment.Methods A total of 114 SLE patients were selected as the study group,and 38 healthy volunteers were selected as the control group.The levels of serum Caspase-1,DKK-1 and IL-2Rαwere compared between the two groups and the study group with different disease course.The correlation between serum Caspase-1,DKK-1,IL-2Rαand clinical indicators(white blood cell count,hemoglobin,lymphocyte count,platelets,erythrocyte sedimentation rate,serum creatinine,24 h urinary protein,anti-double DNA antibody(dsDNA),complement C3,complement C4)and disease degree was analyzed.According to the degree of renal impairment,the study group was divided into 58 cases with normal renal function,31 cases with mild abnormal renal function,and 25 cases with moderate and severe abnormal renal function.The levels of serum Caspase-1,DKK-1 and IL-2Rαwere compared,and the evaluation value of their on the degree of renal function impairment was analyzed.Results The levels of serum Caspase-1 in the study group were lower than those in the control group,and the levels of serum DKK-1 and IL-2Rαwere higher than those in the control group(P<0.05).With the severity of disease and renal function,the serum Caspase-1 level was decreased,and the serum DKK-1 and IL-2Rαlevels were increased(P<0.05).Serum Caspase-1 was positively correlated with white blood cell count,lymphocyte count,complement C3 and complement C4,but negatively correlated with erythrocyte sedimentation rate,anti-DSDNA antibody and disease degree(P<0.05).Serum DKK-1 and IL-2Rαwere negatively correlated with white blood cell count,lymphocyte count,complement C3 and complement C4,but positively correlated with erythrocyte sedimentation rate,anti-DSDNA antibody and disease degree(P<0.05).The AUC of serum
